Unsucessful labour induction in women with unfavourable cervical scores: predictors and management

Abstract: When misoprostol induction fails, Foley catheter is a safe and effective method for cervical ripening. Foley catheter can be the first treatment of choice in post-term nulliparous women or preterm women with Bishop score

“…DiVerent researchers used diVerent volumes to Wll the Foley balloon with diVerent success rates and variable incidences of cesarean sections. Some used 30 ml [6,16] while others used 50 ml [10]. We used 30 ml volume and we had a C.S.…”

“…It is of low cost, easy to get and to apply, lacks systemic side eVects, reversible and can be used in women with prior cesarean sections [8]. The Foley catheter exerts its eVect by disrupting the integrity of amnion-chorion and myometrium interface and cervical collagen stimulating prostaglandin release and rendering the uterus susceptible [9][10][11].…”

“…The study finding also showed that relatively safe methods of induction of labor such as the Foley catheter were not used at the study site. This may be due to the induction protocol of the study facilities probably reinforced the use of oxytocin infusion and misoprostol as the main modes for induction of labor [18][19][20]. (Table 3).…”

“…Originally licensed as an oral treatment for gastric ulcers [27], it is used off-label worldwide in obstetrics [28]. Misoprostol binds selectively to prostanoid receptors, increases intracellular calcium and contracts myometrium [5,22], while also softening the cervix by collagen disintegration and dissolution [7]. As a result, it shortens the induction-to-delivery interval compared to placebo, oxytocin or other induction agents in women with an unfavourable cervix [14].…”
