Understanding Chronic Venous Disease: A Critical Overview of Its Pathophysiology and Medical Management

Abstract: Chronic venous disease (CVD) is a multifactorial condition affecting an important percentage of the global population. It ranges from mild clinical signs, such as telangiectasias or reticular veins, to severe manifestations, such as venous ulcerations. However, varicose veins (VVs) are the most common manifestation of CVD. The explicit mechanisms of the disease are not well-understood. It seems that genetics and a plethora of environmental agents play an important role in the development and progression of CVD… Show more

“…The pathophysiology of CVeD involves a profound remodeling and injury of the venous wall, valve incompetence, microcirculatory changes, hypoxia, and a significant inflammatory response secondary to venous hypertension [252]. Women, especially those who are pregnant, represent a vulnerable population that suffers from CVeD [253]. Due to the differential hemodynamic activities occurring in the cardiovascular system, exposure to different hormonal factors, and mechanical action due to proper growth of the fetus, approximately 40% of women undergo CVeD during pregnancy [252].…”

“…International studies have shown a high prevalence of chronic venous disease (75.2%) [3], with 40% of men and 32% of women suffering from varicose veins. According to Ortega and coworkers [4], the estimated prevalence of chronic venous disease was between 60-80%, and more than 80% of the total population suffered from minor venous disorders [5]. Bihari and coworkers [6] described the prevalence of lower limb varicose veins (57.1%) in Hungary, a value similar to other European countries.…”

“…This fact is explained by disruption of the morpho-functional state of endothelial cells, which leads to an inflammatory cascade with subsequent pathological changes in the walls of the veins. [4,5,6,7]…”
