Objetivo: Evidenciar o caso de uma paciente gestante, com diagnóstico prévio de neurotuberculose (NT), que após cessar o uso de medicações, evoluiu com quadro de alterações neurológicas, sendo considerado recidiva de meningite tuberculosa (MT). Revisar cientificamente e a importância da realização do tratamento adequado. Detalhamento do caso: Paciente gestante, G3P1A1C, em acompanhamento irregular de pré-natal, procurou pronto atendimento com queixa de cefaleia, hiporexia, vômitos e confusão mental. Apresentou diagnóstico prévio de NT e cessou medicações por conta própria. Em internação, evoluiu com hidrocefalia e necessitou de inserção de derivação ventricular externa (DVE), com coleta de líquor em intraoperatório e resolução da gestação. O diagnóstico de recidiva de MT foi realizado em conjunto entre clínica, exames de imagem e alterações liquoricas, e assim, instaurado tratamento. A mesma recebeu alta com encaminhamento e acompanhamento puerperal e neurológico. Considerações finais: É de extrema importância a orientação da gestante com diagnóstico de tuberculose quanto a adesão ao tratamento e a segurança das medicações prescritas. Se o tratamento for feito corretamente, a gestação não influencia no curso da doença, e podemos evitar possíveis complicações e sequelas materno-fetais.
