Успехи современной хирургии ран невозможны без понимания изменений межклеточных и кле-точно-матриксных взаимодействий. В этой связи последние данные о роли тучных клеток (ТК) в различ-ных патологических состояниях заслуживают особого внимания. ТК играют одну из ключевых ролей в репарации тканей. Максимальное их представительство обнаруживается в коже, слизистых оболочках органов дыхания, желудочно-кишечном тракте. ТК активируются сразу после повреждения кожи и участ-вуют во всех фазах раневого процесса. При повреждении тканей или неблагоприятном воздействии из ТК выделяется целый ряд воспалительных медиаторов, протеаз, факторов роста, которые запускают каскад реакций, в том числе воспалительный процесс. Сериновые протеазы эффективно ремоделируют элементы внеклеточного матрикса, что необходимо для перехода к следующей стадии заживления ран. Многофунк-циональность ТК проявляется высвобождением как провоспалительных, так и противоспалительных и иммуносупрессивных цитокинов, что является важным клиническим аспектом, поскольку неправильная тактика лечения способствует хронизации раневого процесса. Синтез коллагена фибробластами и ангио-генез являются результатом сложных молекулярно-клеточных событий с непосредственным участием ТК. На поздних стадиях раневого процесса во время реэпителизации их медиаторы могут стимулировать кера-тиноциты для восстановления эпидермального барьера. Изучение эффектов ТК при медленной регенера-ции, обусловленной сахарным диабетом, ишемией, денервацией тканей, нарушением микроциркуляции открывает перспективы в лечении длительно незаживающих ран при социально значимых заболеваниях. Таким образом, уже известные данные и дальнейшие фундаментальные исследования ТК позволяют рас-сматривать их в качестве биомаркера динамики и тяжести раневого процесса, и как мишень для регуля-ции раневого процесса на разных стадиях. Mast cells are activated immediately after skin damage and are involved in all phases of the wound process. When tissue damage or adverse effects from mast cells are released, a whole arsenal of inflammatory mediators, proteases, growth factors that trigger a cascade of reactions, including the inflammatory process. Serine proteases efficiently remodel elements of the extracellular matrix, which is necessary for the transition to the next stage of wound healing. Актуальность поиска новых патогенетических механизмов формирования неразвивающейся беременности обусловлена высокой частотой встречаемости заболевания и отсутствием тенденции к снижению, несмотря на проводимые современные реабилитационные мероприятия.Цель исследования: обосновать подходы к реабилитационной терапии пациенток, перенесших неразвивающуюся беременность, на основании полученных данных о количественном и качественном составе макрофагов в их децидуальной ткани.Материал и методы. Исследовали децидуальную ткань пациенток с неразвивающейся беременностью и пациенток, прервавших беременность по собственному желанию. Макрофаги определяли при помощи мышиных моноклональных антител (Anti-CD68 antibody [KP1], #ab955, разведение 1:200) и кроличьих моноклональных антител (Anti-CD163 antibody [EPR19518], #ab182422, разведение 1:500) с использованием методики множественного иммуномаркирования.Результаты. При неразвивающейся беременности количество макрофагов возрастает двукратно по сравнению с физиологически протекающей гестацией. Изменяется соотношение макрофагов М1 и М2 в сторону возрастания числа первых. Максимальное количество CD68+ клеток, а также макрофагов с одновременной экспрессией факторов дифференцировки CD68 и CD163 определяется при неразвивающейся беременности, сопровождающейся явлениями децидуита.Заключение. Децидуальные макрофаги участвуют в патогенезе неразвивающейся беременности за счет увеличения их числа и изменения вектора поляризации по классическому пути активации, характеризующегося активной выработкой клетками провоспалительных цитокинов и цитотоксических молекул, участием в реализации Т-хелперных иммунных реакций 1 типа. Патогенетически обосновано назначение препаратов, способных репрограммировать макрофагальный ответ, у пациенток перенесших неразвивающуюся беременность, в составе комплексной реабилитационной терапии. Assessment of Decidual Macrophages Population in Patients with Non-Developing PregnancyThe relevance of the search for new pathogenetic mechanisms of non-developing pregnancy is due to the high incidence of the disease and the lack of a downward trend, despite the current reabilitation measures.The purpose of the study: to substantiate approaches оf rehabilitation therapy patients with nondeveloping pregnancy, based on the data on the quantitative and qualitative composition of macrophages in their decidual tissue.Material and methods. The decidual tissue of patients with non-developing pregnancy and patients who terminated pregnancy at their own will was studied. The aim of research was to study the distribution of mast cell populations in the lungs and spleen depending on their expression of proteases in normal conditions and different periods of simulated hypoxic hypoxia.Material and methods. Hypoxic hypoxia was modeled on 87 outbred male rats using 200 liter chambers. The animals were under experimental conditions for four months. Rats were removed from the experiment after 30, 60, 90 and 120 days. After the animals were removed from the experiment, the lungs and the spleen were removed and fixed in 10% neutral buffer solution of Labiko formalin. Lung and spleen preparations were stained with hematoxylin and eosin and according to Van Gieson. For immunohistochemical studies, a panel of monoclonal antibodies was used: Anti-Mast Cell Tryptase antibody, Anti-Mast Cell Chymase antibody.Results. When modeling hypoxia in the structures of the lungs at the terms of 30 and 60 days, the content of predominantly tryptase-positive cells increased, and by the 90th and 120th days, the content of chymase-positive cells increased. By the end of the experimental exposure, the number of tryptase-positive cells increased by 3 times, and chymase-positive – by 7,7 times compared with the control. As the duration of the experiment increased, the presence of tryptase-positive cells in the spleen increased by 3,5 times, chymase-positive cells – by 7 times in the structures of the red and white pulp.Conclusion. During the formation of chronic hypoxic hypoxia in the body of laboratory animals, there is a redistribution of mast cells expressing tryptase and chymase. The most significant increase in the number of chymase-positive mast cells was noted both in the lungs and in the spleen.
