A consecutive series of 49 women (50 procedures), whose conditions were haemodynamically stable, presenting with acute lower abdominal pain, pelvic tenderness, and either a urine concentration of >50 U/l 3 human chorionic gonadotrophin or a pelvic mass shown by ultrasonography were treated with operative laparoscopy under video monitoring (videopelviscopy) as an alternative to laparotomy.Ectopic pregnancy, ovarian and non-ovarian cysts, pelvic adhesions, endometriosis, and fibroids were found, for which salpingotomy, salpingectomy and salpingo-oophorectomy, cystectomy, adhesiolysis, thermocoagulation, and myomectomy were carried out by laparoscopy. In one patient pelviscopy was repeated because of persistent tubal pregnancy after the fimbria was expressed. Laparotomies were carried out on three patients because treatment was not possible by laparoscopy and on a further patient two days after adhesiolysis had been attempted. These were the only serious complications. For the 46 cases (45 patients) in which operative laparoscopy was successful the mean stay in hospital was 1-9 days after operation, and this group of patients returned to normal activities and to work after an average of 2-3 and 2-6 weeks respectively.Most gynaecological emergencies that are managed by laparotomy can be treated by laparoscopy and benefit both patients and the health service.

Objective: To evaluate the diagnostic, therapeutic and prognostic aspects of ectopic pregnancy at the Regional Hospital of Thiès. Methods and Materials:This is a prospective and descriptive study of patients received for Ectopic Pregnancy (EP) and managed in the gynecology and obstetrics department of the regional hospital of Thiès between 1 July 2013 and 31 December 2014. Diagnostic, therapeutic and prognostic aspects were studied. The data was compiled entered and analyzed using the Sphinx version 5 software. Results:We registered 50 EPs from 695 patients presenting with metrorrhagia in the first trimester, which represents a frequency of 7.2%.The epidemiological profile of the patients was that of a 29 years woman on average, few previous deliveries (28%) and referrals (48%). For the most part it involved a ruptured EP (92%) and of ampulla location (78%). Forty-nine patients (98%) had surgical treatment including 45 salpingectomies (90%). We had one maternal death, representing a 2% mortality rate. Fertility after EP was 33%.Conclusion: EP remains a frequent condition in our hospital. The majority of patients arrive in a state of raptured EP requiring radical surgical interventions

Die aus der Anfangszeit der diagnostischen Laparoskopie mitgeteilten KomplikationenVerletzungen an Darm, Gefäßen und im Urogenitalbereich -haben mit fortschreitender Sicherheit der Operateure abgenommen. Neuere Statistiken belegen aber, daß die rasanten Ausbreitung der operativen Endoskopie mit einem Wiederanstieg schwerer Komplikationen einher geht. Besonders hoch ist die Inzidenz der Verletzungen bei Patientinnen mit vorausgegangenen Bauchoperationen.5 Ursachengruppen werden aufgelistet. An erster Stelle stehen die Darmverletzungen. Schon beim Zugang durch die Bauchdecken können sie entstehen, abhängig von der Indikation zur Laparoskopie, besonders oft bei Second-look-Operationen. Durch Sicherheitsteste und die Verwendung speziellen Insufflationsnadeln und Trokare läßt sich das Risiko vermindern. Auch eine offene Laparoskopie hilft weiter. Intraoperativ sind es vorwiegend strombedingte Schädigungen des Darms. Durch scharfe Präparationen, Ligaturen und Nähte anstelle von Koagulationen im Op-Bereich können diese Komplikationen minimiert werden.Beim Setzen von zusätzlichen Trokaren kann es zu Blutungen in die Bauchdecken kommen, die in der Regel aber problemlos beherrschbar sind. Besonders gefährlich sind Verletzungen der großen Gefäße. Nicht immer führen sie zu Blutungen in die freie Bauchhöhle. Auch ausgedehnte Hämatome im Retroperitoneum können entstehen, die nicht sofort erkennbar sind.Blasenverletzungen entstehen meist intraoperativ beim Koagulieren. Die relativ seltenen, durch Laser oder Strom bedingten Ureterläsionen im Gefolge von Adnexoperationen machen sich oft erst postoperativ nach 2-3 Tagen bemerkbar. Sie können durch exakte und übersichtliche, scharfe Präparation des Ureters, wie am offenen Bauch, vermieden werden. Sehr selten sind Magenverletzungen bei anaesthesiebedingter Überblähung. Sie bedürfen der chirurgischen Versorgung.Eine regelrechte druck-und volumengesteuerte CO 2 -Insufflation bereitet kaum Probleme. Emphyseme entwickeln sich bei fehlerhafter Lage der Veress-Nadel oder des Trokars präperitoneal oder im Netz. Auch CO 2 -Embolien bei intravasaler Insufflation wurden beschrieben. Bei erhöhtem intraabdominalen Druck kann es über vasovagale Reflexe zu Arrhythmien, Hpertensionen und Asystolie kommen. Deshalb ist eine korrekte anaesthesiologische Überwachung wichtig.Bei den endoskopischen Tubensterilisationen hat die Komplikationsrate abgenommen und liegt derzeit im Promillebereich.
