Abstract.A 6-month-old Holstein heifer that was nonresponsive to medical treatment was evaluated for chronic respiratory disease. Complete blood count and serum chemistry revealed neutrophilic leukocytosis and low globulin levels. Assays for bovine leukemia virus, bovine virus diarrhea, and bovine leukocyte adhesion deficiency were negative. Serum globulin subclass assays revealed transient low concentrations of immunoglobulin (Ig) G1 and IgA, persistent low IgG2, and subnormal IgM. Vaccination with 2 doses of multiple, inactived viruses induced seroconversion for most viruses. Flow cytometric analysis of blood lymphocyte subpopulation demonstrated an increase in CD5ϩ B-cells. Blood lymphocyte proliferation and neutrophil function tests were normal. Results of immunologic assays indicated IgG2 deficiency with transient hypogammaglobulinemia.

SUMMARY Primary antibody deficiencies are the most common primary immunodeficiency diseases. They are a heterogeneous group of disorders with various degrees of dysfunctional antibody production resulting from a disruption of B-cell differentiation at different stages. While there has been tremendous recent progress in the understanding of some of these disorders, the etiology remains unknown for the majority of patients. As there is a large spectrum of underlying defects, the age at presentation varies widely, and the clinical manifestations range from an almost complete absence of B cells and serum immunoglobulins to selectively impaired antibody responses to specific antigens with normal total serum immunoglobulin concentrations. However, all of these disorders share an increased susceptibility to infections, affecting predominantly the respiratory tract. A delay of appropriate treatment for some diseases can result in serious complications related to infections, while timely diagnosis and adequate therapy can significantly decrease morbidity and increase life expectancy and quality of life.

IntroduçãoChegando ao final do século XX, o diagnóstico das doenças ainda necessita de uma história clínica detalhada e de um exame físico minucioso, fatos estes que valorizam a prática médica, principalmente, porque a maioria dos pacientes, em muitas áreas do mundo, não têm disponíveis informações adicionais que podem ser fornecidas pelos testes loboratoriais. A ciência moderna e a tecnologia alteram, consideravelmente, a situação nos países industrializados, que dispõem de condições para custearem os altos preços das investigações médico-técnicas 1 . Essas investigações são compostas de exames laboratoriais sofisticados e de diagnósticos por imagem de alta tecnologia.Apresentaremos neste artigo os principais testes laboratoriais para o auxílio no diagnóstico das doenças pulmonares. Esses testes estarão divididos em testes laboratoriais in vitro, testes laboratoriais in vivo e testes de função pulmonar. Testes laboratoriais in vitroGasometrial arterial -mede a tensão dos gases no sangue arterial. Tem como inconveniente o fato de ser um método invasivo, pois exige uma punção arterial. Uma PaCO2 igual ou acima de 40 mmHg sugere a possibilidade do desenvolvimento de insuficiência respiratória aguda e, se encontrar-se igual ou superior a 50 mmHg, sugere que ela é eminente. A maioria dos pacientes com hipercapnia também têm uma PaO2 inferior a 55 mmHg em ar ambiente, sugerindo que o conteúdo de oxigênio do sangue é inadequado para satisfazer as necessidades normais dos órgãos vitais 2,3 . ResumoObjetivo: Os autores apresentam uma revisão a respeito do diagnóstico das doenças pulmonares com ênfase especial nos méto-dos laboratoriais.Métodos: Foi realizado levantamento bibliográrico, através de Medline, e selecionados os artigos de maior relevância sobre o tema proposto.Resultados: O diagnóstico das doenças pulmonares requer avaliação de história clínica, exames laboratóriais, várias técnicas radiológicas, broncoscopia, toracocentese e biópsia pulmonar, dentre outros. Apresentamos os principais exames laboratoriais que incluem o laboratório clínico e o laboratório de função pulmonar para diagnóstico dessas patologias.Conclusão: Os exames laboratóriais constituem parte do diagnóstico das doenças pulmonares. Os resultados obtidos devem ser relacionados com a história clínica e com outros métodos diagnós-ticos. J. pediatr.
