Total glans resurfacing for premalignant lesions of the penis: initial outcome data

Abstract: denuded glans penis is then covered with an extra-genital skin graft. Ten patients underwent TGR: six had recurrent erythroplasia of Queyrat after 5% 5-fluorouracil (5-FU) therapy; one had no clinical response to 5-FU or imiquimod; one had a severe allergic reaction and therefore could not tolerate 5-FU; and two had extensive glans hyperkeratosis and severe dysplasia. RESULTSThere were no postoperative complications. All skin grafts took successfully, and the cosmetic results were excellent. In all cases, path… Show more

“…GR was initially introduced by Bracka in 2000 for the treatment of persistent BXO [25]. In recent years, reports on GR in small series of patients with non-invasive penile cancer have been published, showing promising oncological, cosmetic and functional results [7,15,18]. The largest published series included 25 patients, who maintained a functional penis without compromising oncological control for a mean follow-up time of 29 months [7].…”

“…Total glans surgical resurfacing was reported to be effective in a series of 10 patients, with good cosmesis. 163 Patients and their sexual partners should be counselled and screened for HPV and other sexually transmitted diseases including HIV. The treatment should be conservatively ablative.…”

“…1,2 If only a small portion of the mucosa of the glans is affected by lichen sclerosus or carcinoma in situ, patients can be offered a partial glans resurfacing with excision only of the involved mucosa followed by repair with the use of STG. STGs tend to take better than their full thickness counterpart on the denuded spongy tissue and excellent cosmetic and functional results have been reported in almost all cases.…”

“…STGs tend to take better than their full thickness counterpart on the denuded spongy tissue and excellent cosmetic and functional results have been reported in almost all cases. 2 Partial glansectomy instead is indicated if only a small portion of the glans is affected by squamous cell carcinoma of the penis. In these patients, a wedge including the affected aspect of the glans is excised down to the tunica albuginea in order to achieve adequate clearance and reconstruction is achieved by primary closure of the defect.…”
