Total-Body Cutaneous Examination, Total-Body Photography, and Dermoscopy in the Care of a Patient With Xeroderma Pigmentosum and Multiple Melanomas

Abstract: Background: Xeroderma pigmentosum (XP) is an autosomal recessive disorder characterized by a defect in DNA repair and subsequent increased frequency of cutaneous malignant neoplasms, including melanoma. In patients with XP, patient and family education and aggressive UV radiation protection are the primary means of skin cancer prevention. An important secondary measure in decreasing morbidity and mortality in these patients involves early detection of skin cancers, particularly melanomas.Observations: We descr… Show more

“…The clinical diagnosis of skin tumours at an early stage and the discrimination between malignant lesions and benign lesions is challenging in these patients because of the presence of severe UV‐induced skin damage with the generalized presence of actinic lentigines, AKs and poikiloderma. Recently, dermoscopy has demonstrated to be useful in the clinical assessment and follow‐up of these patients 12,13 …”

“…Recently, dermoscopy has demonstrated to be useful in the clinical assessment and follow-up of these patients. 12,13 Reflectance confocal microscopy (RCM) is a non-invasive diagnostic technique for the study of skin tumours through obtaining optical sections of the lesions at tissular level. 14 During the last 10 years, it has been used for the in vivo assessment of melanocytic and non-melanocytic lesions.…”

“…The efficiency of digital dermoscopy for the detection of early melanomas in high-risk patients, such as in patients with xeroderma pigmentosum (21,22), those receiving anti-BRAF treatment (23)(24)(25), and atypical mole syndrome, has been assessed. Salerni et al (8) in a meta-analysis of digital follow-up concluded the utility of short-term monitoring in patients with atypical mole syndrome.…”

“…However, only five reports with six patients have been reported regarding the utility of dermoscopy for diagnosing skin lesions in XP patients. 1,[5][6][7][8] Here, we report a rare case of XP variant type (XPV) with dermoscopic findings of lentigo maligna. In addition, we conducted a literature review for relevant articles regarding the use of dermoscopy in XP.…”
