Timing Affects the Clinical Outcome of Adjunctive Systemic Antibiotic Therapy for Generalized Aggressive Periodontitis

Abstract: Within the limits of a retrospective analysis, these findings indicate that administration of amoxicillin/metronidazole immediately after initial SRP provides more PD reduction and RAL "gain" in initially deep sites than late administration at SPT with reinstrumentation after 3 months.

“…Kaner et al 45 analyzed the effect amoxicillin + metronidazole immediately after SRP or 3 months after initial therapy (including subgingival reinstrumentation at this stage for this group). The findings, after 6 months, indicated a further PD reduction and greatest gain in CAL on statistically significant in the group that received antibiotics immediately after SRP.…”

“…The recordings of the DMFT score for cariological status, mucosal alterations and temporomandibular dysfunction (TMD) followed the guidelines for oral health surveys of WHO [19]. In addition, a complete clinical periodontal status including probing pocket depth (PD) and clinical attachment level measurement (CAL) from six sites/tooth was performed with a pressure calibrated periodontal probe [20]. Oral hygiene was assessed by dental plaque present or not, according to O’Leary’s plaque control record [21] and bleeding on periodontal probing present or not, according to Ainamo and Bay [22].…”

“…Each patient underwent periodontal pretreatment with systemic administration of Amoxicillin 250-500 mg(Novomox, cipla) eight hourly and with Metronidazole 200-400mg(Metrogyl) eight hourly for five days. [3][4][5][6] Each patient underwent initial therapy with removal of supra and sub gingival plaque and elimination of cofactors (projecting filling margins, carious lesions, etc.). This was supplemented by thorough instruction and motivation aimed at effective oral hygiene.…”
