“…The rationale and technique for resuscitative transverse thoracotomy have been previously detailed [23]. In a trauma patient with injury to the trunk, presenting in extremis, or in circulatory arrest, the Airway-Breathing-Circulation (ABC) sequence is applied.…”

“…Any error, such as entering the wrong intercostal space, like the trainee did, or not performing the incision along the superior border of the rib, but instead crossing several ribs (straight incision instead of a curved incision) leads to a catastrophic waste of time and definitively increases the chances of the patient dying. On the contrary, as previously described [16, 17], the CI is started by a bilateral thoracostomy in the 5th intercostal space on the midaxillary line; these landmarks are well known to any surgeon who knows how to place a chest tube. The incision is then performed laterally to medially, which limits the possibility of crossing several ribs.…”

“…But how should non-experts manage thoracic trauma necessitating a RT, in the absence of an appropriately trained surgeon? Should they opt for the ''classical'' LAT incision trying to find their way in a small exposure incision without prior experience, or should they perform directly the clamshell incision as suggested by others [15,16], trying to compensate for their lack of training by total exposure?…”
