Thoracoscopic pericardial window for management of pericardial effusion in 15 dogs

Abstract: Thoracoscopic pericardial window is of low morbidity with short surgery and hospitalisation times. It provides good long-term control of idiopathic pericardial effusion but short-term palliation of clinical signs in dogs with neoplastic disease.

“…Furthermore, current literature lacks guidelines about the ideal size of such windows. Defects varying from 0.5–1×0.5–1 cm (Sumner-Smith and Archibald 1971), minimum 3×3 cm (Atencia and others 2013), 4×4 cm (Walsh and others 1999, Skinner and others 2014) and 6×6 cm (Crumbaker and others 2010) have been described, irrespective of the patient's size.…”

“…No incidences have been reported within veterinary literature, but in human medicine it is a well-recognised complication (Casalduero and others 2006, King and others 2009). Another reason for the persistent need of an alternative to thoracoscopic pericardiectomy is the limited availability of the equipment and lack of expertise (Atencia and others 2013, Tillson 2015). …”

“…Different techniques have been described for long‐term relief of cardiac tamponade and include both minimally invasive and open approaches to remove varying amounts of the pericardium . Reports have been published describing subxiphoid thoracoscopic pericardiectomies performed with instrument ports on both the left and the right side of the dog . Single side port placements are referenced as an alternative in 2 relevant textbooks .…”
