Objective: The aim of our study was to assess body composition status and its association with inflammatory profile and extent of intestinal damage in ulcerative colitis patients during clinical remission. Method: This is a cross-sectional study in which body composition data (phase angle [PhA] Lean mass was inversely correlated with non-specific inflammation marker (APMt vs. ESR) and directly correlated with hemoglobin values (MAMC vs. hemoglobin). Logistic regression analysis revealed that body cell mass was associated with disease extent (OR 0.92; 95CI 0.87-0.97; p<0.01). Conclusion: PhA was inversely correlated with inflammatory markers and directly correlated with lean mass. Acute inflammatory markers were correlated with disease extent. Body cell mass was associated with disease extent.

Background and Aim: To evaluate the usefulness of a low FODMAP (Fermentable Oligosaccharides, Disaccharides, Monosaccharides, and Polyols) diet on patients with irritable bowel syndrome (IBS), non-active inflammatory bowel diseases (IBD), and celiac disease (CD) on a gluten-free diet (GFD). Methods: Dietetic interventional prospective study. IBS, IBD, and CD subjects were evaluated to check if they fulfilled the Rome III criteria. Each subject was educated to follow a low FODMAP diet after being evaluated by filling out questionnaires that assessed the quality of life (QoL) and symptoms experienced (IBS-SSS and SF-36), and was reevaluated after 1 and 3 months. Results: One hundred twenty-seven subjects were enrolled: 56 with IBS, 30 with IBD, and 41 with CD. IBS-SSS showed that abdominal symptoms improved after 1 and 3 months of diet in all subjects, with significant difference among the 3 groups at T0 (average scores IBS: 293 ± 137, IBD: 206 ± 86, CD: 222 ± 65, p < 0.001), but no difference at T3 (IBS: 88 ± 54, IBD: 73 ± 45, CD: 77 ± 49, p = ns). By analyzing the SF-36 questionnaire, we did not observe any difference between the 3 groups, in terms of response to diet (p = ns), we observed a clinical improvement from T0 to T3 for most of the questionnaire’s domains. Conclusions: A low FODMAP diet could be a valid option to counter abdominal symptoms in patients with IBS, non-active IBD, or CD on a GFD, and thus, improve their QoL and social relations.

La Société Européenne de Gastroentérologie Pédiatrique Hépatologie et Nutrition (ESPGHAN) arécemment publié un appel en faveur d'un meilleur contrôle qualité des probiotiques. Le constate st que « les procédures telles que la fermentation, la composition de la matrice, la récolte cellulaire, le séchage par pulvérisation, la lyophilisation et les conditions de stockage telles que la température, l'humidité et le pH ne sont que quelques-uns du large éventail des facteurs déterminants de la fabrication pouvant affecter la survie microbienne, la viabilité et finalement les résultats de l'étude et/ou les résultats cliniques » [1].C'est un point de vue que je partage totalement et en faveur duquel je milite depuis de nombreuses années. Regardons les choses en face,n ombre de probiotiques aujourd'hui disponibles jouent du flou réglementaire entourant le marché du complément alimentaire pour faire valoir des bénéfices qu'un contrôle plus poussé réduirait à néant.Que dit la réglementation ?P eu de choses en vérité et rien, à l'heure actuelle, ne contrarie l'arrivées ur le marché d'un probiotique dont la composition et le processus de fabrication n'ont pas été testés. Et pour plus de confusion, on généralise volontiers les bénéfices de telle souche, de telle association, en vertu d'une vision simpliste qui consiste à faire croire qu'à composition équivalente, effets équivalents. C'est ainsi qu'apparaissent régulièrement des « clones » de formulations prouvées scientifiquement, qui revendiquent malgré l'absence d'études, les même bénéfices santé.C eci est d'autant plus préjudiciable, qu'il s'agit dans ce cas précis de probiotiques à usage médical, recommandésp ar les sociétéss avantes pour des indications précises. Prenons le cas concret du probiotique VSL#3® que je connais bien puisque c'est sous ce nom que la formulation que j'ai inventéei lyad enombreuses années, af ait l'objet de plus de 60 études cliniques et consécutivement de directives internationales, pour le traitement de la colite ulcéreuse et la pochite [2,3,4].Après interruption de mon partenariat avecl as ociété VSL,j 'ai récupéré l'entière propriété de ma formulation et du processus de fabrication.
