Thiazolidinediones Could Improve Endothelial Dysfunction and Risk of Premature Coronary Heart Disease in HIV-Infected Patients

Abstract: The use of Human Immunodeficiency Virus (HIV) protease inhibitors as part of the Highly Active Antiretroviral Therapy (HAART) is associated with atherogenic dyslipoproteinemia, insulin resistance, hypertension and endothelial dysfunction, all of which contribute to premature coronary heart disease. These abnormalities appear to be associated with an increase in cardiovascular events in HIV-infected patients. Beneficial metabolic effects of anti-diabetic agents used in HIV-infected patients have been reported. … Show more

“…As with metformin there has been evidence showing the benefits of the TZDs in improving insulin sensitivity, fat redistribution and specific CV metabolic indices in HIV lipodystrophy 22, 28. The potential benefits of TZDs, which promote differentiation of preadipocytes to adipocytes and redistributing fat from the viscera to subcutaneous tissue, suggested that they may be ideal agents in treating HIV lipodystrophy.…”

“…Thiazolidinediones, PPAR gamma agonists may improve some of the features of HAART-induced dyslipidaemia and glucose intolerance including reduced circulating glucose, haemoglobin (Hb)A1c and triglycerides concentrations and raised HDL-cholesterol concentrations. 144 Another potential bene¢t is stimulation of peripheral blood mononuclear cells by PPAR gamma agonists such as rosiglitazone which inhibit HIV replication and reduce cytokine production such as TNF-alpha.…”
