Diabetes and HIV: the role of a specialist clinic

Abstract: The rising incidence of type 2 diabetes in the developed world is already well established. There are, however, specific populations in the occident where there is an increase in type 2 diabetes, but unrelated to obesity. One such group is the HIV infected population. Associated with this increase in diabetes, there has been a similar rise in cardiovascular disease. This review article aims to outline the aetiological factors that may precipitate diabetes and cardiovascular disease in this group, and establish… Show more

“…As a result of that, the incidence of 11 diabetes mellitus is increasing in the HIV infected population more than that of the general population, even in the absence of the concomitant problem of obesity (20 (23). The overall prevalence of glucose disorders in HIV-infected patients treated with HAART, which includes diabetes mellitus, impaired fasting glucose and impaired glucose tolerance, was found to be at 25% for that particular study (23).…”

“…This inhibition leads to inflammation and insulin resistance. 13,[16][17][18] NRTIs Treatment with NRTI drugs, in particular, thymidine analogs such as d4T (stavudine), can cause mitochondrial toxicity, which induces fat cell apoptosis or cell death. Elevated free fatty acids and rates of lipolysis lead to lipodystrophic syndrome, and this is associated with insulin resistance, hypertriglyceridemia, low HDL-C, and the metabolic syndrome.…”

“…T2DM prevalence and complication rates in the HIV-infected population (23–40%) are noticeably higher than the general population [9]. Traditional risk factors as well as HIV-specific factors including anti-retrovirals and lipodystrophy syndrome contribute to the pathogenesis [9].…”

Traditional risk factors as well as HIV-specific factors including anti-retrovirals and lipodystrophy syndrome contribute to the pathogenesis [9]. A strong body of evidence supports the use of bariatric surgery as a treatment modality for T2DM in the context of obesity [10] with sustained remission of T2DM described [7].…”
