The Undescended Testis in Adult Life

Abstract: The management of the undescended testis in the adult patient presents problems in terms of locating the impalpable gonad, the risk of malignancy and prognosis with regard to fertility. CT scanning has been used to locate four intra-abdominal testes. The risk of malignancy has been assessed by determining the incidence of carcinoma in situ after orchiectomy or testicular biopsy in 90 patients (112 testes). Carcinoma in situ was found in 4 of 16 abdominal, 3 of 44 inguinal and none of 52 scrotal testes followin… Show more

“…There is a suggestion from some studies that patients who undergo orchipexy before the age of 10 years have a relatively low risk of developing tumours compared with those who undergo correction later (Martin et al, 1979;Strader et al, 1988). It is for these reasons that some authors have recommended orchidectomy for unilateral undescended testis when diagnosed after puberty (Ford et al, 1985).…”

“…Data of 222 cases with known testicular volume, location and pathology results were evaluated separately from 97 cases with known semen analysis. Mean age of patients was 21.26±0.85 and median age was 21 (19)(20)(21)(22)(23)(24). Mean testicular volume of pathological specimens was 6.90±5.81 cc [median 6 cc (0.1-30)].…”

“…For this reason, orchiectomy is indicated in adult patients with cryptorchidism. [15][16][17][18][19] Although recently remarkable developments in the treatment of germ-cell neoplasia have been achieved, together with a decrease in disease-specific and perioperative mortality rates, orchiectomy can be performed in men up to 50 years of age with unilateral cryptorchidism. [20] In the literature, there are several studies about incidence of incidental tumor in adult patients with cryptorchidism who had undergone prophylactic orchiopexy for tumors.…”

“…CIS was found in 4 of 16 intra-abdominal testes compared with 3 of 44 inguinal testes (7%). Ten of the 16 abdominal testes contained Sertoli cells only (Ford et al, 1985).…”
