Background: Accurate and timely cause of death (COD) data are essential for informed public health policymaking. Medical certification of COD generally provides the majority of COD data in a population and is an essential component of civil registration and vital statistics (CRVS) systems. Accurate completion of the medical certificate of cause of death (MCCOD) should be a relatively straightforward procedure for physicians, but mistakes are common. Here, we present three training strategies implemented in five countries supported by the Bloomberg Philanthropies Data for Health (D4H) Initiative at the University of Melbourne (UoM) and evaluate the impact on the quality of certification. Methods: The three training strategies evaluated were (1) training of trainers (TOT) in the Philippines, Myanmar, and Sri Lanka; (2) direct training of physicians by the UoM D4H in Papua New Guinea (PNG); and (3) the implementation of an online and basic training strategy in Peru. The evaluation involved an assessment of MCCODs before and after training using an assessment tool developed by the University of Melbourne. Results: The TOT strategy led to reductions in incorrectly completed certificates of between 28% in Sri Lanka and 40% in the Philippines. Following direct training of physicians in PNG, the reduction in incorrectly completed certificates was 30%. In Peru, the reduction in incorrect certificates was 30% after implementation and training on an online system only and 43% after training on both the online system and basic medical certification principles. Conclusions: The results of this study indicate that a variety of training strategies can produce benefits in the quality of certification, but further improvements are possible. The experiences of D4H suggest several aspects of the strategies that should be further developed to improve outcomes, particularly key stakeholder engagement from early in the intervention and local committees to oversee activities and support an improved culture in hospitals to support better diagnostic skills and practices.

Background Civil registration and vital statistics (CRVS) systems play a key role in upholding human rights and generating data for health and good governance. They also can help monitor progress in achieving the United Nations Sustainable Development Goals. Although many countries have made substantial progress in strengthening their CRVS systems, most lowand middle-income countries still have underdeveloped systems. The objective of this systematic review is to identify national policies that can help countries strengthen their systems. Methods and findings The ABI/INFORM, Embase, JSTOR, PubMed, and WHO Index Medicus databases were systematically searched for policies to improve birth and/or death registration on 24 January 2017. Global stakeholders were also contacted for relevant grey literature. For the purposes of this review, policies were categorised as supply, demand, incentive, penalty, or combination (i.e., at least two of the preceding policy approaches). Quantitative results on changes in vital event registration rates were presented for individual comparative articles. Qualitative systematic review methodology, including meta-ethnography, was used for qualitative syntheses on operational considerations encompassing acceptability to recipients and staff, human resource requirements, information technology or infrastructure requirements, costs to the health system, unintended effects, facilitators, and barriers. El objetivo de este estudio fue describir y analizar el proceso de certificación de muerte y nacimiento de un hospital docente en Bogotá en el año 2014, tanto en términos de flujo de información como de calidad de la causa de muerte asignada. Se diseñó una metodología para monitorear la calidad y cobertura de las estadísticas de nacimientos y muertes hospitalarias. Durante su práctica de salud pública, diecisiete estudiantes de noveno semestre de Medicina participaron en la recopilación de información sobre 511 registros clínicos de muertes no fetales, excluyendo las muertes por una causa externa que ocurrieron en 2014 (58 % de todas las muertes no fetales ocurridas en el hospital ese año). Los estudiantes, previamente entrenados, completaron los certificados de defunción, supervisados por profesores de diferentes especialidades, de acuerdo con el diagnóstico, mediante el acceso a los registros médicos de los fallecidos. La causa básica de muerte asignada después de la revisión se comparó con la causa básica originalmente certificada por médicos del hospital, en el sistema de registro en línea en uso en Colombia: RUAF-ND. Se estimó el acuerdo en la identificación de la causa básica de muerte. Se obtuvo una concordancia bruta del 80 % y un acuerdo de Kappa entre el 57 % y el 95 %, todos superiores, estadísticamente, a cero, al agrupar las causas básicas de muerte por capítulos de la CIE10, con un 6 % de códigos inutilizables en las dos bases. Este estudio podría ser replicado en otros hospitales universitarios y así fortalecer la capacitación de estudiantes de medicina en estadísticas vitales mientras se monitorea la calidad del registro de nacimientos y muertes en hospitales docentes.
