The Temporomandibular Joint in a Rheumatoid Arthritis Patient after Orthodontic Treatment

Abstract: A 32-year-old Japanese female patient consulted the authors' dental clinic with a 4.5-year history of rheumatoid arthritis (RA). She complained of pain during mouth opening and difficulty in eating due to masticatory dysfunction caused by an anterior open bite. Imaging showed severe erosion and flattening of both condyles. RA stabilized after pharmacological therapy and became inactive during the orthodontic therapy aimed at reconstructing an optimal occlusion capable of promoting functional repositioning of t… Show more

“…These include postponing the start of orthodontic-surgical treatment 113 . More recently, pharmacological control has been recommended both before and during orthodontic surgical treatment in order to stabilize active CR 59,84,116 . The different options of treatment include condylectomy and reconstruction with costochondral grafting 49,69,131 ; disc repositioning 143-145 ;alloplastic joint reconstruction 35,38,91 , recommended in cases of advanced condyle destruction (Figure 6).…”

“…It seems that for adults the protrusive function only is not enough to maintain the mandible in the forward position. Sato et al [ 29 ], Sasaguri et al [ 26 ] and Tanaka et al [ 36 ] tried to maintain the mandible in the forward position by changing the occlusal configuration not only anterioposteriorly but also vertically. As a result, obvious remodelling of the condyle in adults has been found [ 26 , 29 , 36 ].…”

“…Condylar remodeling after retention was also reported by Sasaguri et al . [ 14 ] in a case with rheumatoid arthritis, suggesting the remodeling of the eroded condyles might be an adaptative response to a stable and functional occlusion.…”
