ResumoA imagem corporal é amplamente defi nida como o conjunto de percepções, pensamentos e sentimentos de um indivíduo sobre o seu próprio corpo. A alteração da imagem corporal em quadros de obesidade severa é relatada consistentemente na literatura médica e psicológica. O objetivo da presente pesquisa foi investigar o instrumental de avaliação de imagem corporal utilizado em estudos com obesos no contexto de intervenção cirúrgica de redução de peso. Realizou-se uma revisão sistemática em cinco bases de dados no período de janeiro de 2009 a dezembro de 2014. Artigos em língua inglesa e portuguesa foram incluídos para análise. A busca com os descritores retornou 462 registros dos quais 15 cumpriram os critérios de inclusão. Foram identifi cados nove instrumentos que mensuram imagem corporal. Destacou-se a prevalência de instrumentos de autorrelato sobre a satisfação com o próprio corpo. Embora a literatura descreva a alteração perceptiva da imagem corporal em obesos, a avaliação prioriza o nível de satisfação subjetiva com o próprio corpo. Changes in body image in severe obesity condition are consistently reported in the medical and psychological literature. The aim of this research was to investigate the set of instrumental assessment applied in studies of body image in obese within the context of surgical intervention for weight loss. We conducted a systematic review in fi ve databases from January 2009 to December 2014. Articles in English and Portuguese were included for analysis. I denne oversiktsartikkelen drøftes kunnskapsgrunnlaget for postbariatrisk plastikkirurgi og hvilke behandlingstilbud som finnes.KUNNSKAPSGRUNNLAG Artikkelen er basert på et litteratursøk i PubMed med søkeordene «bariatric surgery» AND «plastic surgery», supplert med forfatternes erfaringer fra et stort pasientmateriale.RESULTATER Kroppsformende kirurgi etter store vekttap tilbys hovedsakelig for behandling av medisinsk belastende hudplager. Behandlingen kan bidra til økt livskvalitet og høyere funksjonsnivå. Indikasjon for behandling, valg av plastikkirurgisk metode og grad av komplikasjoner er imidlertid svakt vitenskapelig forankret, og plastikkirurgens egen oppfatning og kliniske erfaring er derfor ofte avgjørende. Mange plastikkirurger setter grensen for kroppsformende kirurgi ved BMI < 28 kg/m 2 . De fleste overvektsopererte har BMI  30 kg/m 2 , og en søknad om kroppsformende kirurgi for disse pasientene blir dermed ofte avslått, med unntak av der det er tungtveiende medisinske årsaker.FORTOLKNING Plastikkirurgiske inngrep kan gi høyere funksjonsnivå og økt livskvalitet. Kunnskapsgrunnlaget for indikasjoner, behandlingsmetoder og resultater bør styrkes gjennom godt planlagte prospektive studier og et pasientregister. Det er spesielt behov for dokumentasjon av behandlingsresultat når det gjelder den store pasientgruppen med BMI  30 kg/m 2 .Kirurgisk vektreduserende behandling (bariatrisk kirurgi) har økt i omfang, og omtrent 3 000 mennesker blir årlig fedmeoperert i Norge (1, 2). Livskvalitet og fedmerelaterte følgesykdommer bedres betydelig etter uttalt vekttap (3), men likevel opplever mange nye fysiske og psykiske plager på grunn av store mengder overflødig hud.I denne oversiktsartikkelen drøftes behandlingsindikasjoner, valg av postbariatriske plastikkirurgiske metoder, resultater og komplikasjoner samt hvilke behandlingstilbud som finnes. Aim: A negative effect of massive weight loss is the development of excess skin. Excess skin causes both physical and psychological problems for those affected and their families. The overall aim of this thesis was to improve the knowledge of the prevalence and associated problems of excess skin in post bariatric patients and to evaluate the outcome of abdominoplasty regarding HRQL. Patients and methods:A literature search was performed to evaluate the quality of evidence for health benefits of abdominoplasty. Thereafter, adult post bariatric patients were sent the Sahlgrenska Excess Skin Questionnaire (SESQ) together with a study specific questionnaire to evaluate their experience with excess skin regarding physical, functional and psychosocial impairments, amount and discomfort regarding the same and requested and undergone body contouring surgery. Further, post bariatric adolescents answered the same questionnaires as the adults, and measurements of excess skin were performed. Finally, post bariatric patients undergoing abdominoplasty answered the Short Form-36 (SF-36), a modified version of SESQ and the EuroQol-5D (EQ-5D) before and one year after abdominoplasty. Prior to the surgery, all study participants were randomly assigned to either undergo or not undergo rectus fascia plication, and measurement of the amount of abdominal excess skin was performed.Results: The quality of evidence of positive health effects for patients undergoing abdominoplasty was very low regarding HRQL. Most of the post bariatric patients, including both adolescents and adults of both genders, reported that they suffered from excess skin in several body parts. The majority of the post bariatric patients also requested body contouring surgery. Abdominoplasty increased the physical function subscale of the SF-36 and all 10 items regarding physical, functional and psychosocial impairments of the SESQ. However, plication did not influence the outcome of abdominoplasty regarding HRQL. In addition, low correlations were found between the measured amount of abdominal excess skin and discomfort with the same or change in the physical function subscale of SF-36 following abdominoplasty. Conclusions:Most of the post bariatric patients seem to suffer greatly from excess skin independent of sex or age, and a majority request body contouring surgery at several body sites. Abdominoplasty appears to improve physical and psychosocial dimensions of HRQL.
