The Maryland bridge retainer: A modification of a Maryland bridge

Abstract: Preservation of the maxillary lateral incisor space after orthodontic treatment in an adolescent patient traditionally requires the use of a retainer with a lateral pontic, commonly referred to as a "flipper," which is minimally effective in the long term. This article illustrates a technique for chairside fabrication of a Maryland bridge retainer for semi-permanent retention. A stainless steel braided palatal wire is bonded to the anterior teeth. A pontic is then built intraorally with flowable composite, usi… Show more

“…11,42 In the treatment of the present case, the missing lateral incisor was to be managed by a procedure promoted by Kravitz. 42 However, a simple and economical treatment alternative using fewer materials was offered, which is especially useful in countries of low socioeconomic status. In addition, unlike the minimally invasive aesthetic procedure recommended by Malgaj et al, 11 the management protocol included a more restorative approach on healthy teeth adjacent to the missing tooth and can be clinically applied without the need for a rubber-dam.…”

“…(Lam WYH, 2019) [18] Preservation of the upper lateral incisor space after orthodontic treatment traditionally requires the use of a retainer with a lateral pontic; A pontic is constructed with fluid composite while the retainer is maintained until the patient is of age for a more permanent restoration. (Kravitz ND, 2020) [17] . Although implants are considered a better treatment option for the rehabilitation of patients with missing anterior teeth, resin-bonded bridges (RBBs) can conservatively restore missing teeth and have therefore gained popularity over the years.…”

“…From a mechanical point of view, these prostheses present a design that is not retentive, and from an aesthetic point of view, when the abutment teeth are very translucent or when there are ample edentulous spaces, the aesthetics may be compromised, since when a metal structure is used, the metal may be visible [2,18]. Therefore, dental practitioners often see RBFPDs as a temporary solution rather than a long-term one [19].…”
