Objetivo: Identificar evidências disponíveis acerca dos cuidados para a prevenção de eventos adversos no transporte intra-hospitalar de paciente crítico adulto. Método: Revisão integrativa de artigos científicos publicados nos últimos 10 anos, através das bases de dados Biblioteca Virtual em Saúde; Medical Literature Analysisand Retrieval System Online; Scopus e Web of Science que respondessem à pergunta: Quais os cuidados para a prevenção de eventos adversos no transporte intra-hospitalar de paciente crítico adulto? A seleção dos estudos foi realizada por dois revisores de forma independente. A extração dos dados foi realizada em instrumento padronizado. Resultados: Dos 640 estudos identificados, nove artigos foram incluídos nesta revisão. Observou-se nos estudos selecionados que o transporte intra-hospitalar traz riscos ao paciente crítico. A implementação de protocolos e o uso de checklists foram as estratégias mais frequentes utilizadas para prevenção de eventos adversos no transporte intra-hospitalar de paciente crítico adulto. Comunicação eficiente e definição de equipe de transporte também foram estratégias apresentadas e discutidas nos estudos. Conclusão: O uso de protocolos e checklists na rotina hospitalar é uma ferramenta importante para a prevenção de eventos adversos decorrentes do transporte intra-hospitalar.

Aim: This study aimed to describe adverse events occurring during intra-hospital transportation of adult patients from the emergency room to the intensive care unit and to analyze the associated risk factors. Design: This study uses a quantitative analytical observational design. Methods: In total, 151 critically ill patients out of 159 with indicated intra-hospital transportation participated in the study. Patients were transported from the emergency room to the intensive care unit in a class A general hospital in Central Java, Indonesia. The statistical analysis included descriptions of demographic, medication, monitoring, equipment, and clinical characteristics of the cohort, and identified risk factors for adverse events during transportation by univariate and multivariate logistic regression analyses. Results: The overall incidence of adverse events was 78.8%. Risk factors were: transport team, hemodynamic monitoring, equipment preparation, and patient's condition, and were associated with adverse events (p < 0.05). Multivariate analysis showed that equipment preparation was the factor most contributing to adverse events with Exp(B) = 22.6. Conclusion: This study showed that the incidence of adverse events during transportation of critically ill patients was high. We recommend that transports be performed by fully equipped teams of medical professionals. Monitoring of intra-hospital transportation-related events is also recommended to reduce frequency of adverse events.

Background. The transportation of critically ill patients presents a precarious situation in which adverse events may occur. At Chris Hani Baragwanath Academic Hospital (CHBAH) patients were manually ventilated using a manual resuscitator bag during transportation from theatre to the intensive care unit (ICU).Objectives. To evaluate the arterial partial pressure of carbon dioxide (PaCO2) levels of ventilated adult critically ill post-operative patients on arrival at the ICU at CHBAH. Methods. This was a cross-sectional study using convenience sampling. Pre- and post-transportation arterial blood gases were obtained from 47 patients. Results. There was a statistically significant difference in the pre- and post-transport PaCO2 level (p=0.03), with a mean difference of 3.3 mmHg. The pre- and post-transport arterial partial pressure of oxygen (PaO2) level (p≤0.001) and the week and weekend pre-transport (p≤0.001) and post-transport (p=0.01) PaCO2 were statistically significantly different. No statistically significant difference was found in the other arterial blood gas parameters or in the post-transport PaCO2 of those patients (26 (55.3%)), who received a neuromuscular blocking drug compared with those that did not. Adverse events were noted during 12 (25.6%) of the transports, 5 (41.7%) of which were patient-related, and 7 (58.3%) of which were infrastructure-related. Conclusion. There was a statistically but not clinically significant difference in the pre- and post-transport PaCO2 level and between week and weekend transportations. Hypercarbia was the most common derangement in all transports. Adverse events occurred during one-quarter of transportations.
