The Free Abdominoplasty Flap and Its Use in Breast Reconstruction: An Experimental Study and Clinical Case Report

Abstract: In search for a free microvascular flap which would give abundant reconstructive material as well as a satisfactory donor site the free abdominoplasty flap was developed. The flap is designed on the area between the umbilicus, the pubic region and the anterior superior iliac spines and is based on the inferior epigastric vessels on one side only. In eight regular abdominoplasty procedures elevation of this flap and keeping the blood flow intact through the isolated vessels did not jeoparding the viability of t… Show more

“…There were other reports on free flap operations, such as the use of silicone breast implants with the groin flap, the opposite side's latissimus dorsi muscle, and the inferior gluteal artery. Since the report by Holmström 4 in 1979 on the composite musculocutaneous flap called the free abdominoplasty flap, which had the deep inferior epigastric artery as its pedicle, and since the development of delicate operating skills, the free TRAM flap operation by using autologous tissue has become the more popular operation in breast reconstruction. Hartrampf and associates 5 presented excellent results using the conventional TRAM flap.…”

“…También es de considerar que la autora utilice la técnica de colgajo TRAM pediculado para la reconstrucción mamaria de forma sistemática, cuando en la actualidad esta técnica se está desechando, salvo en casos muy especiales, dadas las complicaciones que pueden originarse en la pared abdominal, tales como hernias y debilidades (2).…”

“…Since 1979, when Holmström published the first report on the use of the free abdominoplasty flap for breast reconstruction (59), and 1982, when Hartrampf described the use of the pedicled TRAM flap (57), several anatomic studies have been conducted on vascular supply to the abdominal wall (16,17,19,105,188). The rectus abdominis muscle is nourished by the deep superior and inferior epigastric systems.…”
