Although certain risk factors can identify individuals who are most likely to develop chronic pain, few interventions to prevent chronic pain have been identified. To facilitate the identification of preventive interventions, an IMMPACT meeting was convened to discuss research design considerations for clinical trials investigating the prevention of chronic pain. We present general design considerations for prevention trials in populations that are at relatively high risk for developing chronic pain. Specific design considerations included subject identification, timing and duration of treatment, outcomes, timing of assessment, and adjusting for risk factors in the analyses. We provide a detailed examination of 4 models of chronic pain prevention (i.e., chronic post-surgical pain, postherpetic neuralgia, chronic low back pain, and painful chemotherapy-induced peripheral neuropathy). The issues discussed can, in many instances, be extrapolated to other chronic pain conditions. These examples were selected because they are representative models of primary and secondary prevention, reflect persistent pain resulting from multiple insults (i.e., surgery, viral infection, injury, and toxic/noxious element exposure), and are chronically painful conditions that are treated with a range of interventions. Improvements in the design of chronic pain prevention trials could improve assay sensitivity and thus accelerate the identification of efficacious interventions. Such interventions would have the potential to reduce the prevalence of chronic pain in the population. Additionally, standardization of outcomes in prevention clinical trials will facilitate meta-analyses and systematic reviews and improve detection of preventive strategies emerging from clinical trials.

AbstrakNyeri kronis pasca operasi merupakan salah satu nyeri kronis yang paling sering terjadi. Nyeri kronis tersebut memberikan dampak negatif bagi kesehatan mental, kualitas hidup, dan perekonomian masyarakat. Penyebab dari nyeri kronis pasca operasi belum diketahui secara pasti, tetapi diduga bahwa perubahan plastisitas pada sistem saraf pusat berkontribusi dalam terjadinya nyeri kronis. Pencegahan terjadinya nyeri kronis pasca operasi dapat ditempuh dengan beberapa cara yang bertujuan untuk meminimalisir terjadinya sensitisasi pusat di sistem saraf pusat yang mengakibatkan nyeri kronis. Cara yang ditempuh dalam pencegahan nyeri kronis pasca operasi dengan mempertimbangkan faktor psikososial, pemilihan teknik operasi yang dapat meminimalisir kerusakan saraf perifer, pemilihan teknik anestesi, dan penggunaan preemptive analgesia. Dalam penulisan ini bertujuan untuk membahas mengenai patofisiologi nyeri kronis pasca operasi, cara yang ditempuh untuk mencegah terjadinya nyeri kronis pasca operasi, dan perlunya mempertimbangkan faktor psikososial selain intervensi farmakologis sebagai bagian dari pendekatan holistik dari penatalaksanaan perioperatif untuk mencegah nyeri kronis pasca operasi. Day surgery, coming to and leaving the hospital on the same day as surgery as well as ambulatory surgery, leaving hospital within twenty-three hours is increasingly being adopted. There are several potential benefits associated with the avoidance of in-hospital care. Early discharge demands a rapid recovery and low incidence and intensity of surgery and anaesthesia related side-effects; such as pain, nausea and fatigue. Patients must be fit enough and symptom intensity so low that self-care is feasible in order to secure quality of care. Preventive multi-modal analgesia has become the gold standard. Administering paracetamol, NSIADs prior to start of surgery and decreasing the noxious influx by the use of local anaesthetics by peripheral block or infiltration in surgical field prior to incision and at wound closure in combination with intra-operative fast acting opioid analgesics, e.g., remifentanil, have become standard of care. Single preoperative 0.1 mg/kg dose dexamethasone has a combined action, anti-emetic and provides enhanced analgesia. Additional α-2-agonists and/or gabapentin or pregabalin may be used in addition to facilitate the pain management if patients are at risk for more pronounced pain. Paracetamol, NSAIDs and rescue oral opioid is the basic concept for self-care during the first 3–5 days after common day/ambulatory surgical procedures.
