Objective: To determine the effects of combined headgear treatment in high-angle patients and to evaluate the differences 2 years after treatment. Materials and Methods:The study material consisted of lateral cephalograms of 21 patients (13 girls and eight boys; mean age 10 years 3 months) with Class II dental relationship. Only extraoral combined traction that was applied to maxillary first molars was used in the treatment of all patients. The patients were instructed to wear the appliance 20 hours per day until the molar relationship was corrected. The treatment time was approximately 5 months. At the beginning (T0), at the end (T1), and approximately 2 years after (T2) combined headgear treatment, lateral cephalometric radiographs were obtained. Results: SNA and ANB angles and convexity decreased significantly during treatment (T0-T1; P Ͻ .05 and P Ͻ .01, respectively), and the net changes (T0-T2) in ANB angle and convexity were also found to be statistically significant (P Ͻ .01). During treatment, significant distal movement was found in the upper first molar, and the second premolar and molar were also distalized significantly (P Ͻ .001). At the end of 2 years, there was a significant mesial migration of the upper molars and the second premolar during the posttreatment period, but all of them were positioned more distally compared to the beginning of treatment (P Ͻ .001). The decrease in molar relation was statistically significant during the T0-T1 and T0-T2 periods (P Ͻ .001).

Functional appliances are primarily orthopaedic tools used to influence the facial skeleton in a growing child. These appliances may be fixed or removable. A fixed functional appliance referred to as the Repositioning Nance Appliance (RNA) was developed and a case report is presented. The RNA is a fixed functional appliance consisting of bands on the upper first molars joined together with a 0.036-inch stainless steel wire that is bent around the anterior contour of the palate. Just lingual to the upper incisors the wire is complemented with an acrylic Nance button, which is modified with a downward and backward incline projection. The labial aspect of this projection is smooth and when the lower incisors come into contact with it, they are directed into an edge-toedge relation with the upper incisors, brought about by forward positioning of the mandible. This has the net effect of relocating the mandible into a Class I relation. A 14-year-old boy in his late mixed dentition stage of dental development presented with mild crowding, a horizontal growth pattern and a skeletal Class II jaw relation caused by an underdevelopment of the mandible. Treatment was initiated using the RNA and continued for a period of 18 months. This was followed by full fixed appliances for another 12 months and resulted in Class I skeletal and dental relations, a reduced overjet and an improved facial profile. The RNA, if applied correctly is a simple fixed functional appliance that can be used with confidence, comfort and economy to both the patient and orthodontic practitioner.

Objetivo: este estudo cefalométrico longitudinal visou avaliar, comparativamente, as alterações das alturas faciais, provenientes do tratamento da má oclusão de Classe II, 1ª divisão de Angle, pela Técnica de "Edgewise", associado à ancoragem extrabucal cervical, com e sem extração de quatro primeiros pré-molares. Metodologia: a amostra constituiu-se de 116 telerradiografias em norma lateral obtidas de 56 jovens, de ambos os gêneros. Destes jovens, 22 foram tratados ortodonticamente com extração de quatro primeiros pré-molares e apresentavam idade média inicial de 12,30 e final de 14,87 anos, constituindo o grupo I. Outros 22 foram tratados ortodonticamente sem extração e apresentavam idade média inicial de 12,53 e idade média final de 14,73 anos, formando o grupo II. Os demais 14 jovens com idade média inicial de 11,50 e final de 13,63 anos, não foram submetidos a nenhuma intervenção ortodôntica, formando assim o grupo controle (grupo III). Resultados e Conclusões: os resultados demonstraram que não houve influência significante da terapia ortodôntica sobre as alterações das alturas faciais. Os primeiros molares superiores e inferiores desenvolveram-se no sentido vertical nos três grupos. As alterações horizontais dentoalveolares evidenciaram uma mesialização dos primeiros molares superiores e inferiores em todos os grupos, contudo o maior deslocamento para mesial ocorreu no grupo tratado com extração de quatro primeiros pré-molares e no grupo controle. Este fato prova que, no grupo tratado sem extração, a Classe II foi corrigida não pela distalização dos primeiros molares superiores, mas pela restrição da mesialização dos mesmos.
