The Effect of Adjunctive Mitomycin C in Ahmed Glaucoma Valve Implantation

Abstract: Although adjunctive MMC in AGV implantation is safe and effective, it may not offer a better chance of surgical success compared with AGV implantation without MMC.

“…5,16,21,22 Mean IOP was lower in the MMC group during the first postoperative month, but only significantly so at week 3. After this period there was no significant difference in IOP, number of glaucoma medications, success rates, incidence of hypertensive phase, and BCVA among the study groups.…”

“…5,6 A number of studies have evaluated the effect of adjunctive MMC during implantation of nonvalved and valved GDDs on postoperative IOP control. [15][16][17][18][19][20] To the best of our knowledge, this is the first randomized clinical trial simultaneously evaluating the effect of adjunctive AMT and MMC for AGV implantation as compared with standard surgery.…”

“…11,16 Previous work has well established that a fibrous capsule impedes mass transport of fluid to and from implanted devices. [17][18][19] Though the preponderance of groups report no relative improvement in IOP control when using antimetabolites during GDD implantation, [20][21][22][23][24] another study employed both postoperative and extended duration (mean 7.3-7.4 min) intraoperative antimetabolite usage in conjunction with GDD implantation. This group demonstrated improved long-term IOP control rates, possibly through retardation of fibrotic capsule formation.…”

“…This might be because MMC-induced tissue death stimulates a reactive fibrosis around the AGV in very young eyes [13]. In addition, a previous report noted increased postoperative complications such as hypotony and tube exposure in patients who underwent AGV implantation with adjunctive MMC [14]. …”
