Objectives: The vascular supply to the neck and body of the pancreas is highly variable. The dorsal pancreatic artery is the dominant artery feeding this area. The aim of this study was to describe the vascular supply of postresection pancreatic remnants after pancreaticoduodenectomy. Patients with hazardous anatomical arrangement may be at a higher risk of postresection remnant ischemia and postoperative pancreatic fistula development. Methods: The modified Whipple procedure was performed on 20 cadaveric donors. The macroscopic anatomical supply of the postresection pancreatic remnant of each donor was evaluated. Results: The arterial supply of the postresection remnant was highly variable. In 30% of cases (6/20), the dorsal pancreatic artery was cut during the pancreatoduodenectomy or it was missing. In these cases, the area of the pancreaticojejunostomy construction was fed only through anastomoses between the transverse pancreatic artery and the pancreatic branches of the splenic artery. Conclusions: In 30% of cases, the arterial supply of the postresection pancreatic remnant was dependent on inconstant intraparenchymal arterial anastomoses. These patients may be at a higher risk of postoperative pancreatic fistula development.

Purpose The aim of this study is to explore the application value of " retrocolic approach, artery priority, uncinate process first " in laparoscopic pancreaticoduodenectomy, and to elaborate other common approaches. Methods We retrospectively analyzed the clinical data of 100 patients who underwent laparoscopic pancreaticoduodenectomy using this approach in our hospital from January 2018 to October 2018. We used SPSS22.0 for statistical analysis. Measurement data were expressed as OR M (Q25-Q75), and count data were expressed as frequency and rate. Results The average operation time was 267.82 ± 49.18 minutes, the average intraoperative blood loss was 165.30 ± 100.49ml, and 17 patients received blood transfusion. The average time of gastric tube removal was 5.67 ± 2.95 days, and the average time of eating was 5.32 ± 2.79 days. Postoperative complications occurred in 22 patients, including pancreatic fistula (n = 12), biliary fistula (n = 5), delayed gastric emptying (n = 2), intestinal obstruction (n = 1), gastrointestinal fistula (n = 1), postoperative bleeding (n = 8), abdominal infection (n = 9), pulmonary infection (n = 6), and reoperation (gastroscopy) (n = 8). The average length of hospital stay was 16.79 ± 8.98 days, and one patient died (1.00%). The mean tumor size was 3.03 ± 2.41cm. The mean number of lymph nodes dissected was 11.77 ± 5.66, and the mean number of positive lymph nodes was 1.10 ± 1.81. Conclusions The " retrocolic approach, artery priority, uncinate process first " approach shorted the operation time, reduced intraoperative blood loss and surgical complications. It is a safe, rapid and effective approach, which is worthy of further clinical promotion and application.

Интерес к анатомии селезеночной артерии, приходившийся на середину XX в., в конце предыдущего столетия и начале ХХI в. разгорелся с новой силой. Внедрение новых методов диагностики, таких как компьютерная томография (КТ) сосудов, и увеличение хирургических вмешательств на органах бассейна селезеночной артерии, в частности на поджелудочной железе, особенно с применением малоинвазивных, интервенционных и роботизированных технологий, только увеличили интерес к анатомии данного сосуда. Материал и методы. Проведен систематический обзор литературы библиографических баз данных PubMed, Embase, CyberLeninka и GoogleScholar, охватывающих 1910-2021 гг., с использованием следующих ключевых слов и их комбинаций: «чревный ствол», «селезеночная артерия», «селезеночные сосуды», «артерии тела и хвоста поджелудочной железы», «кровоснабжение поджелудочной железы», «анатомия», «варианты» и «аномалии». Результаты и их обсуждение. Описаны и проанализированы все имеющиеся на сегодняшний день случаи врожденного отсутствия селезеночной артерии. По данным анатомических и радиологических исследований за последние 100 лет основным источником селезеночной артерии является чревный ствол, частота данного варианта колеблется от 90,6 до 100 %. По результатам морфометрии длина селезеночной артерии колеблется от 2 до 32 см, а диаметр (калибр) -от 4 до 11 мм. По отношению к поджелудочной железе селезеночная артерия может располагаться супрапанкреатически (63,3-99,33 % случаев), антеропанкреатически (препанкреатически) (4,2-18,5 %), интрапанкреатически (1,2-6,66 %) или ретропанкреатически (2,8-36,6 %). Основными панкреатическими ветвями селезеночной артерии являются дорсальная, большая и хвостовая панкреатические артерии с частотой 24,7-47,2, 45,7-100 и 26-100 % соответственно. Заключение. Систематический обзор литературы показал различия в топографии селезеночной артерии в результатах анатомических и радиологических исследований. Перспективными представляются оригинальные исследования с включением достаточного количества как анатомического, так и радиологического материала, с едиными статистическими подходами и интерпретацией данных строго с учетом международной анатомической номенклатуры.
