ObjectivePatients experiencing unexplained chronic throat symptoms (UCTS) are frequently referred to gastroenterology and otolaryngology outpatient departments for investigation. Often despite extensive investigations, an identifiable structural abnormality to account for the symptoms is not found. The objective of this article is to provide a concise appraisal of the evidence-base for current approaches to the assessment and management of UCTS, their clinical outcomes, and related healthcare utilisation.DesignThis multidisciplinary review critically examines the current understanding of aetiological theories and pathophysiological drivers in UCTS and summarises the evidence base underpinning various diagnostic and management approaches.ResultsThe evidence gathered from the review suggests that single-specialty approaches to UCTS inadequately capture the substantial heterogeneity and pervasive overlaps among clinical features and biopsychosocial factors and suggests a more unified approach is needed.ConclusionDrawing on contemporary insights from the gastrointestinal literature for disorders of gut–brain interaction, this article proposes a refreshed interdisciplinary approach characterised by a positive diagnosis framework and patient-centred therapeutic model. The overarching aim of this approach is to improve patient outcomes and foster collaborative research efforts.

We aimed to analyse voice signals in 40 patients with chronic laryngitis elicited by exposure to chemical factors. We ex- amined 20 people with catarrhal chronic laryngitis (group 1), 20 people with subatrophic chronic laryngitis (group 2) and 15 healthy volunteers as controls. All subjects underwent acoustic examination of the voice signal using the software Praat V 4.2.1. We studied acoustic measures as follows: Jitter, Shimmer and NHR (noise-to-harmonics ratio). The analysis of the obtained data revealed statistically significant differ- ences in the average values of Jitter and Shimmer measures, as well as in the ratio of nonharmonic (noise) and harmonic component in the spectrum ( NHR) in patients with chronic laryngitis (groups 1 and 2) compared with controls. In group 1 (chronic catarrhal laryngitis), the average values of acoustic measures such as Jitter, Shimmer and NHR were as follows: Jitter - 0.92 ± 0.1%, Shimmer - 5.31 ± 0.5%, NHR - 0.078 ± 0.04. In group 2 (subatorophic laryngitis), the average values of acoustic measures were: Jitter - 0.67 ± 0.6%, Shimmer - 6.57 ± 0.7% and NHR - 0.028 ± 0.003. The obtained data indicate a pronounced instability of the voice in frequency and amplitude, a significant proportion of the noise component in the spectrum of the voice signal in the examined patients with chronic laryngitis exposed to chemical factors. The most pronounced alterations were found in patients with catarrhal chronic laryngitis. We conclude that the quantitative values of spectral analysis of the voice signal Jitter, Shimmer, NHR may serve as valuable criteria of the degree of voice impair- ment. This may be helpful in determining the effectiveness of rehabilitation measures.

BACKGROUND: It is known that an increased intake of copper (Cu) has an adverse effect, and above all leads to the defeat of parenchymal organs, including liver tissue. AIM: This study the morphological changes in the hepatic tissue at the impact of polymetallic Cu dust. METHODS: An experimental study was carried out on the outbred white male rats. Dust was injected once intratracheally at a dose of 50 mg. For dynamic observation, the animals were killed in 1, 3, and 6 months with the control group using instant decapitation. The Balkhash industrial polymetallic dust with a predominant Cu content (Cu-0.6%) was used for the study. Morphological changes were assessed using histological and morphometric methods. RESULTS: Morphometric examination of liver tissue at 30 days showed Vv necrosis increasing in 320 times in Group 2 (p < 0.001), Vv infiltrates – in 121 times (p < 0.001), Vv dystrophic altered hepatocytes – in 19.91 times (p < 0.001), Vv dual-core cells – in 23 times (p < 0.01), and Vv fibrosis – in 2.82 times (p < 0.001) in comparison with Group 1. Vv portal tracts are not reliably changed. In 90 days, there were also the following morphometric parameters increasing in comparison with the control group: Vv necrosis – in 522 times (p < 0.001), Vv infiltrates – in 395 times (p < 0.001), Vv dystrophic altered hepatocytes – in 26.7 times (p < 0.001), Vv dual-core cells – in 314 times (p < 0.01), and Vv fibrosis – in 13.27 times (p < 0.001). On the 180 day of the experiment, there was the increasing of Vv infiltrates in 421 times (p < 0.001), Vv dystrophic altered hepatocytes – in 34.09 times (p < 0.001), Vv dual-core cells – in 411 times (p < 0.001), and Vv fibrosis – in 54.09 times (p < 0.001) CONCLUSION: The impact of polymetallic dust with 0.6% Cu concentration at the early stages leads to the changes in the liver in the form of reactive hepatitis with the following transformation into portal-type hepatitis.
