SUMMARY Objectives Insufficient data are available comparing medical costs, hospitalization or admission rate, and drugs used for the treatment of pregnant and nonpregnant women admited to the emergency department (ED) due to suicide attempts following drug intoxication. We sought to evaluate these differences in our ED. Methods This is a prospective non-pregnant women (4.3% vs. 26.2%; p=0.019). Comparison of drug intoxication revealed a higher rate of intake of organophosphate (39.1%; vs. 34.8% p<0.001) and paracetamol (39.1% vs. 19.9%; p=0.031), but a lower rate of multidrug ingestion (39.1% vs. 67.8%; p=0.005) ÖZET AmaçÜlkemizde gebelerde intihar girişimiyle ilgili yeterli çalışma yoktur. Çalışmanın amacı acil servisimize intihar amaçlı oral aşırı ilaç alımı ile başvuran gebe hastalar ile aynı yaş grubunda intihar amaçlı oral aşırı ilaç alımı ile başvuran gebe olmayan kadın hastaların yatış oranlarını, zehirlendiği ilaç tiplerini ve maliyet farklarını karşılaştırmaktır. Gereç ve YöntemAtatürk Üniversitesi Tıp Fakültesi Acil servisine oral ilaç alarak intihar teşebbüsü (parasuisid) nedeniyle başvuran kadın hastalar kesitsel ve prospektif olarak incelendi. 2008 ve 2011 yılları arasında acil servise başvuran her iki grubun tedavi planları, tahmini maliyetleri ve hastane yatış oranları ile ilgili verilerini toplayıp karşılaştırdık. BulgularGebe grubunda 23 (%7.4) hasta ve kontrol grubunda 286 (%92.6) olmak üzere toplam 309 hasta çalışmaya alındı. Gebe hastalarda gebe olmayanlara göre daha önceden psikiyatrik hastalık öyküsü olması anlamlı olarak daha düşüktür (%4.3 ve %26.2; p=0.019). Gebe hastalarda gebe olmayanlara göre organofosfat (%34.8 ve %39.1; p<0.001) ve parasetamol (%39.1 ve %19.9; p=0.031) zehirlenmesi daha sık görülmekte iken çoklu ilaç alımına bağlı zehirlenme daha az oranda görülmektedir (%39.1 ve %67.8; p=0.005). Gebe hastaların acil serviste müşahede oranları gebe olmayanlara göre daha düşük olup (%56.5 ve %77.6), dahili servislere (%13 ve %6.3) ve yoğun bakım ünitesine yatış oranları (%13'e karşı %3.1) daha yüksektir (p=0.039). Buna bağlı olarak tedavi maliyetleri de gebe hastalarda daha yüksektir (2553.5±6206.7 TL ve 581.3±490.3 TL; p<0.001). SonuçGebe olmayan bayan hastalarla karşılaştırıldığında gebelerde ilaç ile intihar teşebbüsü daha nadir görülmesine rağmen, yüksek hospitalizasyon oranları ve yüksek maliyet nedeniyle daha yüksek öneme sahiptir. Bu nedenle girişim öncesi dönemde gebe hasta grubunun intihar riskini belirlemek için acil olarak özel tarama programlarına ihtiyaç vardır.

CONTEXT AND OBJECTIVE: Knowledge of socioeconomic impact of attempted suicide may sensitize managers regarding prevention strategies. There are no published data on this in Brazil. The aim here was to describe the direct and indirect costs of care of hospitalized cases of attempted suicide and compare these with the costs of acute coronary syndrome cases. DESIGN AND SETTING: Cost-of-illness study at a public university hospital in Brazil. METHOD:The costs of care of 17 patients hospitalized due to attempted suicide were compared with those of 17 acute coronary syndrome cases at the same hospital, over the same period. The direct costs were the summation of the hospital and out-of-hospital costs resulting from the event, determined from the medical records. The indirect costs were estimated through the human capital lost. The Mann-Whitney test and analysis of covariance (ANCOVA) with transformation adjusted for age were used for comparisons. RESULTS:The average costs per episode of attempted suicide were: direct cost, US$ 6168.65; indirect cost, US$ 688.08; and total cost, US$ 7163.75. Comparative analysis showed a difference between the indirect costs to family members, with significantly higher costs in the attempted suicide group (P = 0.0022). CONCLUSION:The cost of care relating to attempted suicide is high and the indirect cost to family members reinforces the idea that suicidal behavior not only affects the individual but also his social environment.
