Aim: To compare the functional outcome as well as elbow range of motion (ROM) after triceps splitting and triceps sparing approach for AO/OTA TYPE A distal humerus fractures. Materials and Methods: This is a prospective study done at our center between 2011 and 2014. A total of 50 patients presented with extra articular distal humerus fracture (AO/OTA 13 A2, 13A3). Exclusion criteria removed 16 patients from the study while 2 patients died due to medical comorbidities before the final follow up. Out of the remaining 32 patients, they were divided into two groups: triceps splitting (15 patients) and triceps sparing (17 patients). Elbow ROM, along with Disabilities of the Arm, Shoulder and Hand questionnaire scores, was compared between the two groups. Results: Triceps sparing group had greater elbow flexion (140.0 ± 4.0) compared to triceps splitting group (126.0 ± 10.0) with p < 0.001. Extension contracture was also significantly less in triceps sparing (5.0 ± 6.0) group as compared to triceps splitting group (24.0 ± 8.0) with p < 0.001. However, there was no statistically significant difference in terms of DASH scores between the two groups with DASH score being (24.28 ± 10.14) in the sparing group as compared to (30.41 ± 14.36) in the splitting group with p = 0.169. Conclusion: As compared to triceps splitting approach, triceps sparing approach results in better elbow ROM with less extension contracture, however both approaches result in similar functional outcome.

ReSuMo: A Movimentação Passiva Contínua (CPM) é utilizada na reabilitação após trauma ou cirurgia articular, e sua aplicação pode gerar resultados positivos na reabilitação dos membros. Este estudo teve como objetivo analisar e discutir a utilização de equipamentos de CPM na reabilitação do cotovelo a partir de uma revisão bibliográfica de estudos publicados nos últimos 10 anos. Esta pesquisa bibliográfica foi realizada em banco de dados informatizado da Biblioteca Virtual em Saúde, EBSCO HOST, PeDRO, Biblioteca Cochrane e Science Direct. Para localizar os estudos, foram utilizadas palavras-chave: movimento passivo contínuo, cotovelo e CPM cotovelo, em Português e Inglês. Foram selecionados artigos completos em português, inglês e espanhol. A estratégia de pesquisa estava focalizada em estudos do tratamento de patologias do cotovelo utilizando a CPM como método isolado ou associado a outras técnicas fisioterapêuticas. A amostra foi composta de 16 publicações de periódicos. Em 11 dos artigos, a aplicação da CPM associada ou não a outros métodos fisioterapêuticos gerou resultados benéficos; um não apresentou diferença quando comparada a utilização ou não da CPM; dois demonstraram resultados positivos, ressaltando cuidados específicos; e dois afirmam ser necessário mais estudos para comprovação de resultados. Portanto, foi possível evidenciar que pesquisas estão explorando a utilização da CPM na reabilitação do cotovelo, porém os parâmetros utilizados e os modos de aplicação do equipamento não estão bem especificados. Além disso, foi possível perceber que a CPM está sendo utilizada como adjuvante aos métodos de reabilitação fisioterápica e prevalecem os resultados positivos.

T-type intercondylar fractures of the distal humerus are rare injuries in skeletally immature patients. Few studies are available to guide the treatment of these injuries. Small case series and expert recommendations suggest superiority of the open approach for direct reduction of the articular surface. This approach poses risks of vascular compromise and elbow stiffness. However, success with percutaneous intercondylar screw fixation has been described for simple split patterns. The authors present a case that illustrates a novel percutaneous technique to treat this fracture type. A healthy 8-year-old boy sustained a T-type intercondylar fracture of the distal humerus (AO 13-C1). Excellent reduction was noted after closed reduction and crossed percutaneous pinning. Compression was applied across the pins through a small external fixator bar to address the intercondylar split component. At the 6-week postoperative clinic visit, the pins and fixator were removed uneventfully, and normal healing with full range of elbow motion was noted. This case report describes a simple, minimally invasive technique to treat uncomplicated T-type intercondylar fractures of the distal humerus in skeletally immature patients. Practitioners familiar with percutaneous fixation of supracondylar fractures can easily perform this technique. The described technique spares an open dissection and avoids permanent implants around the joint. Further multicenter research is warranted to determine the long-term outcomes and safety of the described technique. [Orthopedics. 2017; 40(6):e1096-e1098.].
