Abstract:The aim of the study was to investigate the effects of highly cushioned shoes on running biomechanics. Sixteen recreational runners (8 males, 8 females) participated and ran at a self-selected pace across the force platform in the research laboratory wearing either the standard or highly cushioned shoes, in randomized order. Impact peak (IP), loading rate to IP (LR), active peak (AP), contact time (CT), strike index (SI), running velocity, and knee and ankle kinematics at initial contact (IC) and AP were recorded during the running trials. Overall, there was no effect of footwear on IP, LR, AP, CT and velocity (p>.05) with small effect sizes (ES<0.2). The highly cushioned shoes resulted in a more anterior foot strike pattern, based on the slightly higher SI (p=.03, ES=0.5), although the runners demonstrated a rearfoot strike pattern regardless of shoe condition. No kinematic differences were observed at IC or AP, across shoe conditions (p>.05). Our results indicate that the highly cushioned shoes did not show immediate changes in running biomechanics.

The Effects of Various Orthotic Treatments on the Standing and Walking Performance of Diabetic Patients

Purpose: Today considering the high prevalence of diabetes and its associated pressure ulcers, also irreparable consequences such as amputation and death, the use of orthotic devices is increasing. A variety of devices have designed to prevent ulcers in the patients' feet, however other aspects of the treatment, especially the performance of these patients in standing and walking, which are their main daily activities, have not been considered in designing and use of orthotic devices. In addition, the differences between the available devices and their effects have not been described yet. Therefore the aim of this review article is to answer these issues. Methods:A comprehensive research was done in databases of PubMed, Google Scholar, and ISI Web of Knowledge. The keywords such as "diabetic neuropathy", "orthosis", "plantar pressure", "balance", "energy consumption", and "spatial-temporal parameters of gait" and their results were analyzed. The quality of articles were evaluated by Downs and Black tool.Results: After proper search, 245 English articles were found with the related keywords. Then, 184 articles were excluded because of a common title or incomplete text. Also, 25 articles were excluded from the study, because their contents did not relate highly to this study or have reasonable conclusion based on the findings of other articles. Eventually the results of 36 articles were examined and reported. Conclusion:This study has examined the various articles from randomized control trial to case report. Most studies emphasized on the reduction of plantar pressures and ulcer treatment with orthotic devices. A few studies are available on the effects of orthosis in standing balance and walking parameters. However, no study was found with regard to the effect of orthotic treatments on energy consumption in these patients. Because this topic is very important, it is suggested that future studies be focused on these issues.

Для повышения эффективности лечения и реабилитации пациентов, страдающих остеоартрозом, часто рекомендуется использовать ортопедические стельки. Лечебный эффект в этом случае достигается за счет уменьшения аддукционного момента, воздействующего на биомеханическую ось нижней конечности, бла-годаря особой конструкции обуви, параметры которой определяются с учетом биомеханических параметров локомоции пациента. Однако к настоящему времени в научных источниках отсутствует достаточно убедитель-ная информация о том, как влияют различные параметры конструкции обуви на нагрузку коленного сустава. Методология: видеоанализ ходьбы при использовании обуви с различной высотой каблука, уровнем переката носка и пятки. Результаты. В исследовании принимали участие 15 здоровых мужчин. Исследование показало, что аддукци-онный момент возрос при ходьбе в обуви с каблуком 5,5 см, а также с увеличенным перекатом пятки и углом носка p < 0,05. Ортопедическая обувь 3CR, которая включает все вышеупомянутые факторы, в значительной степени повысила суммарный медиальный момент и аддукционный момент колена p < 0,05. Заключение: использование неправильно подобранной обуви и ортопедических средств в течение длительного времени может существенно способствовать развитию остеоартроза коленного сустава.Ключевые слова: остеоартроз коленного сустава, аддукционный момент, видеоанализ, кинетика, конструкция обуви. The effect of different footwear profiles on knee adduction moment have not been fully studied. Methods. Fifteen healthy volunteer subjects, age 25.3 (±2.73), undertook a series of gait laboratory trials with adapted shoes. Kinematic and kinetic data were collect using 16 Oqus 3+ cameras and the walking speed was controlled using timing gates. High street shoes were adapted to include five different heel heights (varying from a 1.5 cm to 5.5 cm heels), two heel profile conditions (curved and semi-curved heels), three varying apex angles (10, 15, and 20 degrees), and barefoot and 3CR footwear conditions. The baseline shoe had no heel curve, a heel height of 3.5cm, an apex position of 62.5% of the shoe length, an apex angle of 15 deg, and a rigid forepart of the shoe. Results. The shoe with 5.5 cm heel height significantly increased the mean knee adduction moment during 50%-100% of the stance phase compared to the 1.5 cm heel (p = 0.008). The high heel shoe also significantly increased knee adduction impulse (area under the curve) versus the 1.5, 2.5, and 3.5 cm heels, and the 10° toe angle and barefoot condition.
