Systematic review with meta‐analysis: the impact of a depressive state on disease course in adult inflammatory bowel disease

Abstract: There is limited evidence to support an association between depressive state and subsequent deterioration in disease course in IBD, but what data that exist are more supportive of an association with CD than UC. Baseline disease activity may be an important factor in this relationship. Further studies are needed to understand the relationship between mental health and outcomes in IBD.

“…Yet levels of anxiety and depression have been found in up to 66% of IBD cohorts . Moreover, stress and maladaptive coping styles are found in increased frequency in patients with IBD, are associated with reduced quality of life, and are strong predictors of disease activity and physical and psychological symptoms even among patients in clinical remission . In our study, the value of the decision support tool in facilitating psychological risk assessment was highlighted in its identification of a patient with suicidal ideation, which would have been missed without the standardised prompts of the decision support tool.…”

“…reported that depression was not associated with flare of disease activity during longitudinal follow-up. 62 However, only four studies were included in the formal meta-analysis. The authors identified seven other studies, five of which did demonstrate a significant association between antecedent psychological disorders and subsequent disease activity.…”

“…This pathology requires surgical intervention in 16 and 47 % of NUC and CD patients, respectively, every 10 years after the onset of the disease. As a result, patients with chronic IBD experience a significant decline in the quality of life, compared to healthy people of the same age [1]. According to recent epidemiological studies, the annual incidence of NUC in Europe is 10.4, and CD -5.6 cases per 100,000 population, with the largest gradient observed in Scandinavia (24.5 and 9.2 cases per 100,000, respectively).…”
