Surgical Transapical Approach for Prosthetic Mitral Paravalvular Leak Closure: Early Results

Abstract: The objective is to demonstrate safety and early clinical results of surgical transapical closure of paravalvular leaks (PVLs) following mitral valve replacement in significant regurgitation. Between March 2014 and February 2015, 12 patients (mean age 52.1 ± 6.0 years, 66.6% male) with severe symptomatic mitral PVLs (n = 13) underwent surgical transapical closure procedure through left mini-thoracotomy. All patients were in NYHA functional class III-IV and median logistic EuroSCORE was 24.2 ± 6.4% (range, 13.5… Show more

“…erefore, the transapical puncture procedure could also be safe. However, there is a potential risk of accidentally puncturing the left anterior descending coronary artery [27][28][29]. So, a selective coronary angiogram is necessary to guide the puncture.…”

“…The high risk of intraoperative mortality ruled out the patient as a surgical candidate. Furthermore, in their study, because the patient suffered clinically insignificant embolic complication and had no vegetation or paravalvular leakage, they did not insist on removing the valve [ 29 , 30 ] . Lejko-Zupanc et al [ 12 ] defend that in selected cases, particularly those with few complications or minor vegetation, clinical treatment should be attempted first.…”

“…Recently a new device (Occlutech PLD Occluder, Occlutech, Jena, Germany) for PVL closure has been developed and introduced into clinical practice. To date, only a few studies of mitral PVL treated by the surgical transapical catheter-based approach have been published [ 21 23 ].…”
