Psoriasis is an immune-mediated inflammatory skin disease with a complex etiology involving environmental and genetic factors. Psoriasis patients often require long-term treatment. Shanyaotianua decoction (STT), a typical traditional Chinese medicine prescription, positively affects psoriasis, although its molecular targets remain unknown. To elucidate its molecular mechanisms, a combination of network pharmacology, bioinformatics analysis, and drug similarity comparisons were employed. Participants were separated into 3 groups: non-lesional (NL), lesions after medication (LM), and psoriasis lesion groups (LS). Based on the Gene Ontology/kyoto encyclopedia of genes and genomes enrichment analyses, the key targets were mainly enriched for biological processes (immuno-inflammatory responses, leukocyte differentiation, lipid metabolic disorders, and viral infection) with the relevant pathways (Janus kinase/signal transducers and activators of transcription and adipocytokine signaling and T-helper 17 cell differentiation), thus identifying the possible action mechanism of STT against psoriasis. Target prediction for 18 STT compounds that matched the screening criteria was performed. Then, the STT compounds were intersected with the differentially expressed genes of the psoriatic process, and 5 proteins were potential targets for STT. Based on the open-source toolkit RDKit and DrugBank database, and through molecular docking and drug similarity comparisons, spinasterol, diosgenin, and 24-Methylcholest-5-enyl-3belta-O-glucopyranoside_qt may be potential drugs for psoriasis.

Psoriasis (Greek. Psora, the itch) [1] is a chronic, relapsing papulosquamous disorder of undefined etiology, characterized by localized or generalized, mostly symmetrical, sharply demarcated plaques and papules, usually covered with white or silver scales [2] on the scalp, elbows, knees, lumbosacral area and in the body folds. Prevalence of psoriasis in countries vary between 0.09% [3] and 11.4%. [4] Psoriasis can occur at any age, and is most common in the age group 50-69. [5] It is considered equally prevalent in both sexes. [2,6] A seasonal variation of the disease, with a higher frequency of flares in winter and spring, has been seen. [7] The prevalence of Psoriasis varies among different ethnic groups and geographical locations, more common in colder northern zone compared to tropical zone.Clinical Presentations: Psoriasis affects skin and nails, along with associated arthritis and systemic diseases. The most frequently reported symptoms are scaling of the skin(92%), itching, erythema, fatigue, swelling, burning and bleeding. [8] Scale is characteristically silvery white and can vary in thickness. Removal of scale may reveal tiny bleeding points (Auspitz sign). The common clinical types of Psoriasis are summarised in the International Psoriasis Council classification and consist of plaque psoriasis, guttate psoriasis, pustular psoriasis, erythrodermic psoriasis, eczematous psoriasis, and photosensitive psoriasis. [9] Plaque type Psoriasis, also known as Psoriasis vulgaris, is the commonest form representing 70-80% of psoriatic patients. [10] Histology also varies among the different clinical variants. An identification of two subtypes of Psoriasis based on the "age of disease onset" has been described by Henseler and Christophers; Type I or Early Onset Psoriasis, presents at ≤ 40 years of age and Type II or Late Onset Psoriasis, developing after the age of 40. Multiple studies have shown that these two subtypes are clinically, genetically and immunologically heterogeneous. Early onset psoriasis, in contrast to Late onset psoriasis, is probably hereditary and shows HLA-Cw6 positivity and parental history with an irregular course, stronger tendency to become generalized, [7] more nail involvement and greater psychosocial impact. Background: Psoriasis is a common, chronic inflammatory skin disease characterized by scaly white papules and pinpoint bleeding on scratching. Presence of keratinocyte hyperproliferation and abnormal differentiation in the epidermis are some significant features. Histopathologically, it is difficult for the dermatopathologists to differentiate psoriasis from psoriasiform dermatitis when there is a lack of typical features. Aims: To study the Ki-67 antigen expression in the different layers of epidermis of psoriatic skin lesion and its utility in the diagnosis and to differentiate psoriasis from other psoriasiform dermatitis by studying the distribution pattern of Ki-67 immunostaining. Methods: In this cross-sectional study, a total of 91 skin lesions which were clinically labelled as psoriasis and psoriasiform dermatitis were studied, which was confirmed by histopathological examination and followed by Ki-67 immunostaining. The distribution of Ki-67 immunostaining in the supra-basal layer, basal layer and whole epidermis was studied. Results: Ki67 staining was significantly higher in the suprabasal layer and whole epidermis in psoriatic lesions compared to psoriasiform dermatitis. The suprabasal Ki-67 mitotic index was also significantly higher in psoriasis group than psoriasiform dermatitis (p <0.05). We found that in psoriasis > 50% Ki-67 positive keratinocytes are scattered in the suprabasal layer of the epidermis in comparison to the psoriasiform dermatitis which is < 50%. Conclusion: We suggest that Ki-67 labelling index can be used for diagnosing psoriasis and also can differentiate it from other psoriasiform dermatitis.
