La fatigue est un des effets secondaires du cancer les plus courants, et pourtant les cliniciens y accordent généralement peu d'attention dans le cadre des rendez-vous cliniques souvent fort chargés. Ce projet visait à évaluer les propriétés psychométriques d'un nouvel instrument à deux éléments conçu pour identifier rapidement les patients qui éprouvent des difficultés liées à la fatigue. Nous avons réalisé l'éva-luation auprès de 190 patients atteints d'un cancer du poumon et traités dans des cliniques ambulatoires. Le pictogramme de la fatigue a affiché une bonne fiabilité de test-retest sur une période de 24 heures (coefficients kappa pondérés de 0,71 pour la première question et de 0,72 pour la seconde) et sur le plan de l'équivalence des méthodes (en personne et au téléphone) (coefficients kappa pondérés de 0,64 pour la première question et de 0,65 pour la seconde). Nous avons évalué la validité en comparant les résultats du nouvel outil avec l'inventaire multidimensionnel de la fatigue et le questionnaire SF-36. Dans l'ensemble, les patients qui affichaient des niveaux de fatigue élevés ont indiqué des niveaux élevés sur toutes les échelles respectives. Le nouveau pictogramme de la fatigue était facile à administrer et à coter dans un milieu clinique fort occupé. Il fournit un instrument standardisé fiable et valide permettant d'identifier les patients qui éprouvent des difficultés liées à la fatigue.La fatigue est un des effets secondaires du cancer qui survient le plus souvent et qui cause le plus de détresse. Il s'agit du seul symptôme signalé par la plupart des patients atteints d'un cancer, et ce, dans tous les groupes de diagnostic (Curt et Johnson, 2003). La fatigue réduit la qualité de vie et restreint de nombreuses activités de la vie quotidienne (Ahlberg, Ekman, Gaston-Johansson & Mock, 2003). Malheureusement, elle n'est pas souvent évaluée de façon systématique par les prestataires de soins, ce qui signifie que ces derniers sont souvent peu conscients de l'impact de la fatigue sur leurs clients (Davis, Lai, Hahn & Cella, 2008). De plus, on offre peu d'interventions de routine pour aider les patients à gérer la fatigue ou à composer avec ses effets, malgré l'existence à un nombre croissant de données probantes sur les effets bénéfiques des interventions en matière de fatigue. Les directives de pratique du National Comprehensive Cancer Network (NCCN) indiquent qu'une approche systématique est nécessaire pour dépister, évaluer et gérer la fatigue de façon appropriée (NCCN, 2007).Les cliniciens recensés mentionnent que les milieux cliniques occupés, les charges professionnelles lourdes et le manque d'instruments d'application simple permettant d'identifier les patients qui éprouvent de la fatigue sont les raisons pour lesquelles cet effet secondaire n'est pas évalué de façon régulière lors des consultations cliniques.
