Sulfadiazine-induced nephrolithiasis in children

Abstract: Sulfadiazine-associated urinary calculi have been described in HIV-positive adult patients but rarely in children. We report two pediatric cases of sulfadiazine-induced nephrolithiasis and review 45 adult cases from the literature. One had a hyper-IgM syndrome and was treated with sulfadiazine for cerebral toxoplasmosis, the other had toxoplasmic retinitis. Both developed multiple bilateral lithiasis with acute renal failure. Normalization of renal function and reduction of calculi size was rapidly achieved af… Show more

“…The accepted treatment regimen for toxoplasmosis is pyrimethamine and sulfadiazine (33). Pyrimethamine treatment requires supplementation with folinic acid to prevent bone marrow suppression, and sulfadiazine can cause severe kidney disease in children (15) and HIV/AIDS patients (7) and can cause rashes after long-term treatment (49). These data highlight the need for the development of new anti-Toxoplasma therapies that are more effective and have fewer adverse side effects.…”

“…3 Recommencement of sulfadiazine therapy is not contraindicated provided there is a high urine flow rate and urine alkalinization (pH >7.15) with monitoring of renal function and urinalysis for crystals.…”

“…Over-dosage of sulfonamides can cause allergic reactions, antibiotic resistance, and carcinogenic effects (Hasebe and Osteryoung, 1975). Because of poor water solubility, sulfonamides can also lead to urinary tract lithiasis such as renal calculus and vesical calculus (Catalano-Pons et al, 2004). Therefore, quick systematic monitoring of sulfonamides in body fluids is an important analytical task.…”

“…Sulfonamides are broad-spectrum and low-cost synthetic antibiotics used in treatment of diseases such as urinary tract infections, pneumocystis pneumonia, chronic bronchitis, meningococcal meningitis, and toxoplasmosis (Catalano-Pons et al, 2004;Joseph and Kumar, 2010). Over-dosage of sulfonamides can cause allergic reactions, antibiotic resistance, and carcinogenic effects (Hasebe and Osteryoung, 1975).…”
