BackgroundSuicidal behaviours are major public health concerns worldwide. They are associated with risk factors that vary with age and gender, occur in combination, and may change over time. The aim of our study was to investigate how frequently patients visiting a hospital emergency room (ER) require a psychiatric consultation for attempted suicide, and to outline the characteristics of this population.MethodsDeterminants of emergency room visits for psychiatric reasons were studied prospectively from 2008 to 2011 at the “Maggiore” Hospital in Novara.Results280 out of 1888 patients requiring psychiatric consultation were referred to the ER because of suicide attempt. Suicide attempters were more often female. The rate of suicide attempters among Italian people was 14.2%, compared to 19.5% in foreigners. Subjects living with parents or own family and those having a permanent job had a higher frequency of suicide attempt. Suicide attempts were more frequent among patients with a history of psychiatric disorders; nonetheless, suicide attempts were more common among those who had not previously been hospitalized in a psychiatric ward or were not under the care of a psychiatrist. The multivariate analysis found that female gender was a risk factor for suicide attempt, while being in the colder months of the year and, surprisingly, unemployment were protective factors.ConclusionsA better understanding of patients referring to the ER due to attempted suicide may allow the identification of at-risk subjects and the implementation of targeted treatment approaches.

resumo objetivo: Considerando o envenenamento como o método mais utilizado para a tentativa de suicídio e a escassez de evidências nacionais sobre o tema, investigamos alguns possíveis indicadores de risco nesse tipo de tentativa. métodos: Estudo do tipo caso-controle em uma emergência geral de um hospital público, na cidade do Recife com 220 indivíduos, distribuídos em dois grupos de 110 pacientes cada, que estavam em tratamento, sendo o grupo casos os sobreviventes de tentativa de suicídio por envenenamento e os controles, sem história de intoxicação/envenenamento nem tentativa de suicídio, pareados por gênero e idade. resultados: O gênero feminino predominou na amostra (70,9%), com idade média de 29 anos; 73% declararam etnia branca ou morena; menos da metade vivia em convívio marital; a maioria tinha religião; ambos tinham poucos anos de estudo. Houve diferença significativa (p = 0,003) para dependência financeira entre os grupos, com chance 2,25 vezes maior para tentar suicídio entre os casos. Ter sofrido fatos traumáticos e abuso sexual na infância revelou diferença significativa. Conclusões: Foram considerados indicadores de risco no grupo caso: estar em dependência financeira de terceiros, ter sofrido abuso sexual na infância, ideação suicida, histórico de transtorno mental na família, possuir algum transtorno mental e, principalmente, comorbidade(s) psiquiátrica(s). No modelo de regressão, foi possível estimar uma chance de tentativa de suicídio por envenenamento de até 94,0% na presença conjunta de quatro fatores. A pesquisa representa uma das primeiras iniciativas para ampliação das discussões sobre os fatores de risco para tentativa de suicídio em âmbito nacional. abstraCt objective: As poisoning is most often used method of attempted suicide and data on this topic are scarce in Brazil, we investigated possible indicators of risk for this type of attempt. methods: A case-control study was conducted at the emergency ward of a public hospital in the city of Recife (Brazil) with 220 individuals divided into two groups, each with 110 participants. ÖzetAmaç: Bu çalışma bir yıllık süre içinde yoğun bakım ünitemizde (YBÜ) ilaç alımı (medikal veya tarım ilacı) ile özkıyım girişiminde bulunan olguları ve ilaca ulaşım yollarını incelemektir. Yöntemler: Özkıyım girişiminde bulunan 50 olgunun (32'si kadın, 18'i erkek) demografik verileri ve tıbbi özellikleri prospektif olarak kaydedilmiştir. Bulgular: Yaş ortalaması 25,4±8,8 olan olguların %66'sı 15-24 yaş arasındaydı. Olguların %28'inde çoklu ilaç kullanımı mevcuttu. İşsizliğin kadınlarda daha fazla olduğu saptandı (p<0,001). Erkeklerin özkıyım için kullandıkları ilacı %27,8 oranında eczaneden aldıkları saptanır iken kadınlar için bu oran %6,2 idi (p=0,03). Olguların %86'sı (n=43) ve kadın olguların %93'ü (n=30) ilacı evdeki ilaç deposundan kullanmışlardı. Eczaneden ilaç alan olguların evdeki ilacı kullanan olgulara göre istatistiksel olarak anlamlı düzeyde daha fazla toksik düzeye ulaştığı saptandı (p=0,06). Toksik düzeye ulaşan hastalarda yoğun bakım ünitesinde kalış süresi toksik düzeye ulaşmayanlara göre istatistiksel olarak anlamlı bulundu (p=0,001). Sonuç: İlaç alımı ile takip ettiğimiz olguların çoğu evlerinde bulunan ilaçlardan ve tarım ilaçlarından faydalanmışlardı. Methods: We prospectively recorded the demographic characteristics and medical data of 50 patients (32 females and 18 males) who were hospitalized in our intensive care unit due to drug-related suicide attempt. Results: 66% of subjects were between the ages of 15 and 24 years with a mean age of 25.4±8.8 years. Multiple drug overdose was observed in 28% of patients. The unemployment rate was higher for women than men (p<0.001). Men obtained drugs from pharmacy more often than women (27.8% vs 6.2%, p=0,03). Of the patients, 86% got the drugs from home medicine cabinet while this rate was 93% among women (n=30). The level of toxicity was higher among patients who got the drugs from pharmacy than in those who used drugs at home (p=0.06). The length of stay in the intensive care unit was longer among patients with toxic level of medication (p=0.001).
