BACKGROUNDThere has been enough evidence that HDL cholesterol plays a major role in reverse cholesterol transport and thereby reduces coronary artery atherosclerotic plaque burden and quantitatively low HDL cholesterol increases coronary artery plaque burden. Recent evidence suggests that in diabetic patients, there is functional impairment in HDL cholesterol which increases the risk of coronary artery disease.So, the present study is conducted in order to compare the severity between diabetic and non-diabetic group of patients with coronary artery disease and to correlate with HDL-C values.The objectives of the study were to study the correlation between quantitative HDL cholesterol level and angiographic severity of coronary artery disease and to compare these findings between diabetic and non-diabetic group of patients. MATERIALS AND METHODS90 Patients with acute ST segment elevation myocardial infarction diagnosed on ECG findings and admitted in ICCU of KIMS Hospital, of which 45 patients had diabetes mellitus.Based on angiographic findings, coronary artery disease severity was calculated using revised BCIS-1 Jeopardy score, and the data obtained was analysed by different statistical methods. RESULTSOut of 90 patients in the study, Mean HDL-C in patients with and without T2DM was 44 mg/dl and 43 mg/dl respectively (p value 0.47). In non-diabetic patients HDL-C was negatively correlated to BCIS1 Jeopardy score (r=0.65, p<0.001). In patients with T2DM there was a positive correlation between HDL-C and BCIS1 Jeopardy score (r=0.38, p= 0.01). CONCLUSIONFrom this study we can conclude that HDL-C though is thought to be protective cholesterol, appears to be appropriate in patients without T2DM. In patients with poorly controlled T2DM, HDL C levels does not show protective role in our study. This could be attributed to the functional impairment of HDL-C in patients with poorly controlled diabetes mellitus. KEYWORDSST Elevation MI; Type 2 Diabetes Mellitus; HDL-Cholesterol. Background: Youth-onset type 2 diabetes (T2D) is increasing in many countries, creating large personal and societal burdens. While many primary health-care professionals (HCPs) are aware of the classic symptoms of T2D, there are several other manifestations that could indicate its presence. Summary: This narrative review summarizes information on these symptoms and indicators, focusing on those less well known. The classic symptoms and comorbidities include frequent urination, excessive thirst, metabolic syndrome, and obesity. In addition to these, the presence of dermatological (e.g., acanthosis nigricans, granuloma annulare, necrobiosis lipoidica diabeticorum, and scleredema), gynecological (e.g., polycystic ovary syndrome, oligomenorrhea, and vulvovaginitis), hepatological (e.g., nonalcoholic fatty liver disease), and psychiatric diseases (e.g., psychosis, depression, and autism) could indicate that a patient has T2D or is at increased risk of T2D. Other less well-known indicators include abnormal blood tests (e.g., oxidized lipids, inflammation markers, hepatokines, and adipokines), prescriptions for antipsychotic medications or statins, and disrupted sleep patterns. Key Message: Due to the diversity of T2D manifestations in young people, primary HCPs need to remain alert to its possible presence.

Chronic inflammation in HIV disease is sustained by immune reconstitution that results from use of combination therapy by patients and manifested by increased expression of inflammatory cytokines. Chronic inflammation in HIV disease is associated with endothelial dysfunction of the cardio vasculature giving rise to reduced elasticity resulting in high blood pressure. This state is marked by increased IL-6 cytokine expression as manifested in patients with type 2 diabetes and hypertension. This study sought to establish cytokine expression in HIV seropositive patients with hypertension comorbidity and correlate with HIV/AIDS progression. We hypothesized that chronic inflammation and presence of hypertension comorbidity presents higher levels of expression of selected interleukins namely IL-6, IL-8 and IL-17A. This study demonstrated that the IL-6 cytokine expression have a positive correlation with the release of IL-8 cytokine as patients with higher IL-6 cytokine values were more likely to have higher IL-8 cytokine levels. This shows that a synergistic effect from both cytokines may accelerate the development of atherosclerosis on blood vessels. Therefore routine measurement of these cytokines may increase prediction of risk of development of hypertension in patients on HIV care.
