OBJECTIVE: The aim of the present study is to conduct a comparative analysis of oral actinic cheilitis and squamous cell carcinomas by immunohistochemical analysis. METHODS: This cross-sectional study utilizes a convenience sample obtained from the archives of an oral pathology laboratory from a higher education institution. Tissue sections from patients diagnosed with actinic cheilitis and carcinoma were immunohistochemically analyzed using monoclonal antibodies specific for smooth muscle alpha actin (SMA) and neuron-glial antigen 2 (NG2). Individual cells that stained positive for these proteins were counted in specific fields by means of histomorphometry. RESUlTS: Subsequent statistical analysis revealed a higher number of NG2-positive when compared with SMA-positive cells in both lesion types (p = 0.005) and these cells were located adjacent to blood vessels. The differences between normal oral mucosa, severe dysplasia, and carcinoma specimens were statistically significant (p = 0.009; p = 0.006). CONClUSION: These data strongly suggest that increased expression of NG2-and SMA-positive cells is associated with formation of blood capillaries, highlighting the importance of angiogenesis in tumor progression.Key words: Carcinoma; Pericytes; Myofibroblasts Expressão imuno-histoquímica de pericitos e miofibroblastos na matriz extracelular de queilite actínica oral e carcinoma de células escamosas: estudo comparativo REsUMO OBJETIVO: O objetivo do presente estudo foi realizar uma análise comparativa da queilite actínica e do carcinoma de células escamosas por meio de análise imuno-histoquímica. MÉTODOS: Este estudo transversal utilizou uma amostra de conveniência obtida dos arquivos de um laboratório de patologia oral de uma instituição de ensino superior. As secções de tecido de pacientes diagnosticados com queiliteactínica e carcinoma foram analisados pela técnica imuno-histoquímica usando anticorpos monoclonais específicos para alfa actina de músculo liso (SMA) e do antígeno neurônio/glial2 (ng2). As células com marcação positiva para estas proteínas foram contadas em campos específicos por meio de histomorfometria. RESUlTADOS: análise estatística subsequente revelou maior número de positivos para ng2 quando comparados com células SMA positivas em ambos os tipos de lesão (p = 0,005) seno que estas células foram localizadas adjacentes aos vasos sanguíneos. As diferenças entre mucosa oral normal, displasia grave e espécimes de carcinoma foram estatisticamente significativas (p = 0,009; p = 0,006). CONClUSÃO: Os dados sugerem, fortemente, que o aumento da expressão de células positivas para ng2 e SMA está associado à formação de capilares sanguíneos, destacando a importância da angiogênese na progressão tumoral.

Introduction. Abnormal angiogenesis is described in tumor growth and it facilitates its metastatic spread. Tumors with high angiogenic activity belong to the category of aggressive tumors with poor prognosis for patients. The aim of this study was to determine the blood vessels density (BVD), i.e. neovascularization at the tumor invasive front in skin squamous cell carcinoma (SCC) in order to determine its possible role in the tumor progression, and to correlate it to the blood vessels density of healthy skin and with the prognostic parameters of the TNM classification: T status, depth of tumor invasion (DI) and tumor histological grade (G), which were also correlated between each other. Material and Methods. The material consisted of surgical specimens obtained from 30 patients with skin SCC, who underwent surgery. Tissue samples were routinely processed by standard paraffin technique stained by Hematoxilin-Eosin and immunohistochemically with antibodies against smooth muscle actin (SMA) and CD34. The BVD in the invasive front of the neoplasms was correlated to the healthy skin, tumor status (pT), depth of invasion and grade of histological differentiation (pG). Results. The histological analysis has shown a high statistical difference in the density of blood vessels in SCC compared to the healthy skin and statistical difference in BVD in neoplasms with different depth of invasion and different grade of differentiation. The density of neovascularzation increased with the deeper invasion and the worse differentiation. Conclusion. The increased vascularization at the invasive front of SCC with deeper invasion and worse differentiation has pointed out to its possible role in neoplasm progression.

Background:Tumor cells work in close coordination with stromal elements from its stage of emergence to metastasis. The study was designed to assess the presence and distribution pattern of stromal fibrocytes and myofibroblasts in oral squamous cell carcinoma (OSCC). Possibility of using these stromal cells as a marker for invasion and lymphnode metastasis was evaluated.Materials and Methods:A total of 40 cases of OSCC consisting twenty cases of each lymph node positive (pN+) and lymph node negative (pN0) samples and ten normal oral mucosa (NOM) tissues were subjected to double immunostaining using CD34 and alpha-smooth muscle actin (α-SMA) antibodies. Stained sections were evaluated semiquantitatively.Results:CD34 fibrocytes were seen in 70% of NOM and none of OSCC samples. α-SMA myofibroblasts were seen in 80% of OSCC and none of NOM samples. A statistically significant difference was found in fibrocyte values (P < 0.001) and myofibroblast values (P < 0.001) between NOM and OSCC study samples. No statistical significance in myofibroblast values between pN0 and pN+ study groups; however, their distribution pattern appreciably varied.Conclusions:This study suggested that fibrocytes could be used as one of the markers for early invasion. Abrupt loss of fibrocytes at the transition zone toward carcinoma and statistical significance in their values supported this inference. Heterogeneity in the distribution pattern of myofibroblasts in tumor stroma indicates that this variability may predict the tumor behavior toward nodal metastasis rather than their mere presence or absence.
