Background: Periodontitis is an inflammatory disease of the supporting tissues of the teeth that leads to crucial changes in the bony architecture. Early detection is of great practical significance so that treatment may be administered before irreversible destruction could occur. Differences exist between different diagnostic methodology concerning the relationship between the measurements and the level of the lesion. The aim of this study was to assess relationship between clinical attachment level (CAL) and alveolar bone crest level (ABCL), reliability of transgingival bone probing level (TBL) for ABCL measurement, and clinical biologic width (CBW) measurement in chronic periodontitis patients. Methods: An analytical cross-sectional study was conducted on 238 periodontal sites in 25 chronic periodontitis patients, aged 20–60 years who were planned for periodontal flap on residual pocket depth of 5-8mm after four weeks of non-surgical therapy. Stent was used for coronal fixed reference point. The relative attachment level (RAL), relative transgingival bone probing level (RTBL), relative alveolar bone crest level (RABCL), and CBW were measured at six sites per tooth excluding third molars. Descriptive and inferential statistical analysis was done. Probability of significance (p-value) was set at 5%. Results: Overall RAL, RTBL and RABCL (mean ± SD) were 8.08 ± 1.23, 10.82 ± 1.1, and 11.15 ± 1.36 mm respectively. Overall CBW (mean ± SD) was 3.06 ± 0.634 mm. Conclusions: There was a significant positive correlation of CAL and TBL with ABCL. Also, the CBW was greater than that of standard histologic BW.

BackgroundThis 6‐month randomized split‐mouth and placebo‐controlled clinical trial aimed to evaluate the clinical and radiographic efficacy of adjunctive use of 0.05% zoledronate (ZLN) gel as local drug delivery to scaling and root planing (SRP) in stage III, grade B periodontitis patients with and without controlled type‐2 diabetes mellitus (DM).MethodsA total of 120 infrabony sites were divided into two groups: Group‐1 (non‐diabetic periodontitis) and Group‐2 (periodontitis + DM). A total of 60 sites in each group were randomized to receive treatment with SRP + placebo gel (control) or SRP + 0.05% ZLN gel (test). Plaque index (PI), modified sulcus bleeding index (mSBI), pocket probing depth (PPD), and relative attachment levels (RAL) were assessed at baseline, 3 and 6 months, and digital intraoral periapical and cone‐beam computed tomography imaging were used to measure the linear and percentage reduction of intrabony defect depth (DD, DDR%) after 6 months.ResultsGroup‐1 showed significant reduction in PI (0.56 ± 0.15 and 0.52 ± 0.19 from 0.67 ± 0.17), mSBI (0.7 ± 0.60 and 0.47 ± 0.57 from 0.9 ± 0.48), PPD (4.6 ± 0.85 and 3.43 ± 0.63 from 6.5 ± 1.04) and gain in RAL (7.03 ± 0.85 and 5.93 ± 0.69 from 8.9 ± 1.09) in the ZLN‐treated sites than the placebo sites and also from Group‐2 sites after 3 and 6 months, respectively. A significant reduction in DD of 28.79% in Group‐1 and 22.20% in Group‐2 at ZLN sites was seen compared to placebo sites of both groups.ConclusionZLN gel applied subgingivally in infrabony pockets resulted in significant clinical improvements evident by probing depth reduction and gain in attachment levels along with radiographic evidence of more bone fill seen in non‐diabetic patients compared to diabetic periodontitis patients.

Aim: The main focus of the study is to determine the difference between the efficacy of xenogeneic collagen matrix (XCM) versus platelet-rich fibrin (PRF) adjunct to coronally advanced flap to treat the gingival recession. Background: Because of esthetic concerns or root hypersensitivity, typical necessity dictates for buccal gingival recession treatment. This study was planned to evaluate and compare the efficacy of XCM with PRF in the treatment of the recessed gingiva. Materials and Methods: Thirty-four sites with Cairo's RT1 and RT2 gingival recessions were taken, out of which 17 sites received XCM (test) and 17 sites received PRF (control). A periodontal treatment was performed first, followed by a re-evaluation. All clinical measurements, including recession height (RH), recession width (RW), clinical attachment level (CAL), keratinized tissue width, and keratinized tissue thickness, were taken before surgery (baseline) and at a 6-month interval following periodontal surgery. Landry's healing index was also measured at the 1 st and 2 nd weeks after surgery. Results: None of the acquired data implies a significant difference statistically for CAL, RW, RH, thickness, and width of keratinized tissue (WKT) at 6-month interval between both the groups. Comparison of healing at the 1 st and 2 nd week intervals among both the groups showed no statistical significance. No significant difference among both Groups A and B was seen according to the intergroup analysis in terms of periodontal parameters such as CAL ( P = 0.374), RW ( P = 0.542), RH ( P = 0.890), WKT ( P = 0.877) and thickness of keratinized tissue ( P = 0.547), and Landry's healing index ( P = 0.429). Conclusion: In consideration of the patient's comfort and the method's simplicity, it can be concluded that PRF or XCM can be employed as an alternative.
