Status Epilepticus Due to Tiagabine Ingestion

Abstract: Tiagabine, in excess dosing scenarios, has been rarely documented to cause status epilepticus. We describe such a case that was not responsive to benzodiazepines, but only to propofol infusion.

“…Tiagabine Increases MC Seizures before GTC Seizures. Tiagabine has been associated with MC seizures or absence status epilepticus in rodent models of generalized epilepsy (Coenen et al, 1995;Hosford and Wang, 1997;Lancel et al, 1998), in individuals with epilepsy (Eckardt and Steinhoff, 1998;Ettinger et al, 1999;Mangano et al, 2003;Skardoutsou et al, 2003;Koepp et al, 2005), and in nonepileptic individuals in overdose (Zhu and Vaughn, 2002;Leikin et al, 2008). In DS mice, we observed a substantial, dose-dependent increase in the number of MC seizures before the first GTC seizure with TGB (Fig.…”

“…A formal analysis of frequency related to tiagabine exposure (Fig. 2), together with reports of tiagabine precipitating NCSE in patients with non-epileptic attacks [13], in an adolescent following accidental ingestion [14], and in patients where TGB was used as a mood stablizer off label [15,16], leaves the author in no doubt that this is real. There has been some discussion about the distinction between a tiagabine toxic encephalopathy and tiagabine-induced NCSE, in particular because of a gradual on and offset and sometimes poor electroclinical response to benzodiazepines [13].…”
