State of the Art in Platelet Function Testing

Abstract: Platelets perform many functions in hemostasis but also in other areas of physiology and pathology. Therefore, it is obvious that many different function tests have been developed, each one conceived and standardized for a special purpose. This review will summarize the different fields in which platelet function testing is currently in use; diagnostics of patients with bleeding disorders, monitoring patients' response to anti-platelet therapy, monitoring in transfusion medicine (blood donors, platelet concent… Show more

“…The latter category comprises PFA 100 and Cone and Plate(let) analyzer (R-Impact) [38]. However, it remains difficult to study platelets in vitro, given that their manipulation can induce activation [39]. Platelets are stored in a mixture of plasma and additive solution with citrate as anticoagulant, which is quite different from their physiological environment.…”

“…More importantly, in vitro test results are often unable to predict platelet function after transfusion, because a certain degree of functional recovery may occur [42,43]. Finally, study results should be analyzed while keeping in mind the possible lack of correlation between the results and clinical efficacy [38,39,43,44].…”

“…Platelet function tests are performed in a variety of patients, including those with bleeding disorders. In most cases, a platelet-mediated hemostatic disorder cannot be established by a single function defect; but, rather, a combination of platelet function abnormalities are required [5]. Applications of platelet function tests are listed in Box 15.1.…”

“…Platelet assays, evaluating platelet adhesion under static or flow conditions and platelet spreading have been developed [107,150] . Platelet adhesion tests in static condition, using a large number of different surfaces -glass beads, cultured vascular cells, purified matrix proteins or complete subendothelial extracellular matrix from cultured endothelial cells -that goes to the detriment of univocal results and standardized procedure, might be achieved [151] .…”
