Confocal laser scanning microscopy (CLSM) is a modern imaging technique that enables the in vivo or ex vivo characterization of skin lesions located in the epidermis and superficial dermis with a high quasi-microscopic resolution. Currently, it is considered to be the most promising imaging tool for the evaluation of superficial skin tumors. The in vivo mode adds the advantage of noninvasive, dynamic, in real-time assessment of the tumor associated vasculature and inflammation. It offers the possibility to repeatedly examine the same skin area without causing any damage and to monitor disease progression and treatment outcome. Furthermore, this novel technology allows the evaluation of the entire lesion and can be used to guide biopsies and to define tumor margins before surgical excision or other invasive therapies. CLSM diagnostic features may differentiate between the various histologic subtypes of skin tumors and therefore helps in choosing the best therapeutic approach. In this study, we present the CLSM characteristic features of the most common melanocytic and non-melanocytic skin tumors, as well as future possible CLSM applications in the study of experimental skin tumorigenesis on animal models.

IntroductionLa dermatoscopie est une technique d'examen non invasive, permettant de donner un nouveau regard de la morphologie clinique des lésions pigmentées et des tumeurs cutanées. Nous montrons à travers notre série les caractéristiques morphologiques dermotoscopiques du carcinome basocellulaire (CBC) chez notre population.MéthodesNotre étude est une étude prospective unicentrique étalée sur une période de 2 ans. Nous avons utilisé le dermoscope chez tous les patients ayant un CBC. L'analyse statistique était réalisée à 'aide du logiciel SPSS version 17.RésultatsOn avait recensé 100 CBC, L'âge moyen des patients était de 51,87 ans, avec un sex ratio F/H = 0,6. Le visage était la localisation la plus fréquente et la plupart des patients étaient de phototype III et IV. On a distingué des critères dermatoscopiques classiques et non classiques. On a montré qu'il existe une relation significative entre le phototype et le degré de la pigmentation des CBC.ConclusionDans ce présent travail, le dermoscope était bénéfique d'une part pour détecter les CBC de petite taille, d'autre part pour faciliter le diagnostic des CBC pigmentés.

Базально-клеточный рак кожи (БКРК) является актуальной проблемой в дерматоонкологии в связи с высокими показателями заболеваемости, разнообразием клинических форм и частыми рецидивами после любых методов лечения. В России оценить реальную заболеваемость можно, только опираясь на показатели рака кожи (исключая меланому) и принимая во внимание факт, что около 70% в этой группе занимает БКРК. В последнее десятилетие в диагностический стандарт активно вошел метод дерматоскопии, позволяющий с высокой степенью вероятности установить данный диагноз даже на самых ранних стадиях развития опухоли. В период 2015-2017 гг. на базе ФГБУ «Приволжский федеральный медицинский исследовательский центр» Минздрава России были обследованы 214 больных от 38 до 89 лет с различными формами БКРК (всего 295 элементов): поверхностной-103, нодулярной-79, пигментной-48, склеродермоподобной-46, фиброэпителиомой Пинкуса-19. Дерматоскопические критерии БКРК были сопоставлены с патоморфологическими признаками, что позволило подтвердить высокую чувствительность и специфичность дерматоскопии при диагностике данного вида опухоли. С учетом описанных дерматоскопически-морфологических корреляций можно констатировать, что различным типам опухоли соответствует собственная дерматоскопическая картина. Дерматоскопическая вариабельность БКРК определяется сочетанием различных факторов, таких как патоморфологический тип, локализация новообразования, пол, возраст, фототип. Комплексный анализ клинических, дерматоскопических и патоморфологических критериев БКРК позволяет с высокой точностью устанавливать диагноз, проводить дифференциальную диагностику, определять факторы высокого и низкого риска рецидивирования, что в свою очередь определяет адекватную тактику ведения пациента. Цель исследования-обоснование возможности использования дерматоскопического исследования в определении степени риска рецидивирования БКРК с последующим выбором тактики лечения. Ключевые слова: базально-клеточный рак кожи, диагностика, дерматоскопические признаки, патоморфологические характеристики, факторы высокого и низкого риска рецидивирования.
