Skin changes caused by d-penicillamine treatment of arthritis. REPORT OF THREE CASES WITH IMMUNOLOGICAL FINDINGS

Abstract: Summary The clinical, histological and immunopathological details of three cases with a d‐penicillamine‐induced eruption are presented. The skin changes included (1) pemphigus erythematosus, (2) pemphigus foliaceus, (3) a bizarre localized pruritic papular eruption, the immunopathology of which was characteristic of lupus erythematosus and (4) fragility due to loss of dermal collagen. Circulating IgA was abnormally low in two cases and circulating immune complexes were demonstrated in the third case.

“…The unique aspect of our case was the observation of variable IF patterns in the skin at different times over a 9-month period. The variety of IF patterns found may not be surprising in view of the fact that penicillamine may induce pemphigus vulgaris, pemphigus erythematosus, pemphigus foliaceus (Hewitt, Benveniste & Lessana-Leibowitz, 1975;Sherak et a/., 1977;Kennedy et al, 1978), and also S.L.E. and positive ANA tests (Camus et al, 1975).…”

“…Most of the subsequent reports have been in RA patients, which reflects the greater use of the drug in treatment of that disorder [30,53,813. The literature refers to pemphigus vulgaris [373, pemphigus foliaceus [-56], and pemphigus erythematosus [30], in relation to penicillamine therapy.…”
