Single-Incision Robotic Colectomy (SIRC) case series: initial experience at a single center

Abstract: Our experience has shown that SIRC can be a safe and feasible procedure for both benign and malignant disease. Patient selection is the key to improving surgical outcomes in SIRC.

“…There has been no specific published evaluation of SIRC's performance in malignancy, including its efficiency with resection margins and the number of lymph nodes harvested. In our own case series , all cancer cases had satisfactory resection margin and lymph node harvest, with an average lymph node yield of 27 (range 17–53). Systematic reviews examining both LESS colectomy and robotic multiport colectomy consistently show that the harvested lymph node numbers are generally satisfactory and comparable to laparoscopic surgery.…”

“…While equivalent clinical outcomes have been reported for robotic colectomies and conventional laparoscopic colectomies [5], single-incision robotic colectomy (SIRC) is hypothesized to further minimize incision-related complications and improve cosmetic outcomes [2]. Aided by the explosive growth of literature in minimally-invasive surgery and technological advances developed by the industry to accommodate specific needs of the procedure, surgeons deem modern SIRC a safe and feasible procedure [6][7][8] with long-term outcomes pending to be determined.…”

As per our results, the mean time for completion of a SILSG procedure should typically be around 70 min, in the hands of experienced laparoscopic surgeons.…”

“…Following laparoscopic exploration of the abdomen, the robot was successfully used for the incision as well as performance of a two layered anastomosis. Additionally Juo et al [48] have published their experience with robotic single incision colorectal resections for different indications. They reported on 31 right hemicolectomies, 20 sigmoid colectomies, 5 left hemicolectomies, 2 low anterior resections and 1 total colectomy.…”

“…Postoperative complications occurred in 27.1% of the cases. Five of those were classified severe complications, three moderate, and seven mild complications [48] . With more and more experience in colorectal robotic surgery, development to total robotic operations with performance of intraabdominal anastomoses has gained more attention.…”
