Single-center experience with the Tigertriever device for the recanalization of large vessel occlusions in acute ischemic stroke

Abstract: BackgroundTigertriever (Rapid Medical, Yoqneam, Israel) is a new design of stent retriever.PurposeTo evaluate the feasibility, safety, and efficacy of the Tigertriever in patients with acute ischemic stroke who have undergone mechanical thrombectomy.Materials and methodsTwo different techniques—namely, standard unsheathing (SUT) and repetitive inflation-deflation (RID) techniques, were used. Modified Thrombolysis in Cerebral Infarction (mTICI) scores of 2b and 3 were considered as successful recanalization.Res… Show more

“…11 Our registry demonstrated a good final reperfusion rate (eTICI 2 b/3) with 94% that is higher than most of the commonly used thrombectomy devices 7,8 or latest generation of hybrid-stent retriever such as NeVa (93%) 12 or the EmboTrap II (76%, Neuravi/ Cerenovus), 13 and higher than the results of the recent retrospective Tigertriever study. 4 However, in 20% an additional rescue device had to be used. Furthermore, the functional outcome with 83% of the patients having a mRS 2 at follow-up demonstrated the efficacy of this device and is similar to others.…”

“…Regarding safety, sICH and mortality rate revealed comparable results to other thrombectomy devices (0-5.3%) 1,14 and is in-line with a recent retrospective Tigertriever study. 4 Limitations include the relatively small sample size.…”

“…Rapid technological developments have led to the substitution of older-generation MT devices by new equipment which use a novel construction of the stent structure (6). One of the newest devices is the Tigertriever (Rapid Medical, Yoqneam, Israel) (7,8), of which there is scarce clinical data on its safety and efficacy (7,9). Tigertriever "is a stent retriever comprising a collapsible, nondetachable, fully retrievable, wire braided construction attached to a 180 cm pusher wire.…”

“…The operator can expand and contract the mesh to conform properly to the diameter of the affected vessel wall. Because the wires of the mesh are completely radiopaque, the device can be seen in its entirety under fluoroscopy" (7). These properties may enable better control of stent opening, resulting in an improved apposition to the vessel wall with adjustable radial force, that in effect, may improve the results of mechanical thrombectomy (Figures 1, 2).…”
