Simultaneously performed off-pump coronary artery bypass grafting and colectomy: a case report

Abstract: This is written so as to report the case of a 71-year-old male with a diagnosis of sigmoid adenocarcinoma accompanied by severe coronary artery disease and unstable angina, which was subject to simultaneous surgical treatment. The patient initially underwent an off-pump coronary artery revascularization in order to avoid the complications of cardiopulmonary bypass, providing the opportunity of a colectomy at the same time with the use of safe surgical means. Our case suggests that performing an off-pump bypass… Show more

“…Our results showed that the off-pump bypass combined with excision of the tumor fully reflected its therapeutic advantages for these patients, neither resulting in the immune perturbation nor in delaying earlier resection of the malignant tumor. Additionally, the results of simultaneous operations involving OPCABG and tumor resection was favored by abundant literature in the past decades [Dedeilias 2010;Darwazah 2011]. Earlier resection of malignant tumor might be beneficial for a patient's survival, corresponding with our result that cancer stage was a factor that affected the long-term survival of patients with the two diseases.…”

“…Cardiac operation combined with pulmonary and esophageal surgeries all are thoracic procedures; this kind of simultaneous surgery could only affect the thoracic organs. However, cardiac operation combined with non-thoracic surgery, such as gastroenteric, urinary and other solid tumor resection surgeries, need extra surgical incisions and may affect the organ function of the corresponding system, while it was poorly reported [Zhao 2017;Dedeilias 2010]. Additionally, the comparison of long-term survival about E622 simultaneous surgery being conducted in CAD patients suffering from malignant tumor involving different organs also is rarely reported.…”

“…Additionally the doubling of costs has to be considered, as well as the advantage in avoiding a second procedure. In the last two decades abundant literature has favoring the results of simultaneous operations involving CAD and cancers [ 1 , 2 , 7 , 8 , 11 , 18 ]. The early and mid-term outcomes of combined neoplastic resection and cardiac operation have demonstrated that this approach is both feasible and safe in carefully selected patients [ 18 ].…”

“…Our results are in agreement with the latter as well as many other studies from the literature demonstrating similar successful operations and good prognosis. Recent studies have demonstrated the favorability of OPCAB over CABG under CPB in simultaneous operations [ 8 , 11 ], however CPB has been shown to affect neutrophils and platelets and results in complement activation, which may be beneficial in patients with malignancies [ 18 ]. From the many clinical investigations published, it appears clear that patients undergoing OPCAB tend to receive a lesser number of bypass grafts [ 19 ].…”
