Objectives: This study aims to investigate the frequency of fibromyalgia syndrome and to specify fibromyalgia syndrome-associated clinical symptoms in patients with acne vulgaris. Patients and methods: Eighty-eight patients (28 males, 60 females; mean age 23.2±5.1 years; range 18 to 40 years) with acne vulgaris and age, sexand body mass index-similar 76 healthy controls (14 males, 62 females; mean age 24.5±2.9 years; range 18 to 35 years) were included. Acne vulgaris was evaluated by using the Global Acne Scale, while Hospital Anxiety and Depression Scale was used to evaluate anxiety. Results: Fibromyalgia-associated pain, sleep disturbance, anxiety, and menstrual cycle disturbance were significantly more frequent in patients with acne vulgaris than controls. Also, the severity of anxiety and the number of tender points were significantly higher in the acne vulgaris patients than controls. Conclusion: This study indicates that patients with acne vulgaris have increased frequency of fibromyalgia syndrome than healthy controls (21.6% versus 5.3%, respectively).

RESUMENLa fibromialgia (FM) es un síndrome de dolor musculoesquelético, idiopáti-co, crónico, de baja prevalencia en la población general, que afecta más a mujeres que a hombres en una relación 9:1; sin embargo, su incidencia en el género masculino va en aumento y el cuadro clínico se caracteriza más por trastornos del sueño que por el clásico dolor generalizado. Por consiguiente, su sospecha clínica en este género es muy baja. Este síndrome con frecuencia se encuentra asociado a fatiga, síntomas depresivos y disfunción sexual, aunque no se cuenta con evidencia suficiente que correlacione esta entidad con antecedentes de trauma testicular y alteración de la relación testosterona libre -globulina transportadora de hormona sexual masculina (SHBG).Presentamos el caso de un paciente masculino de 40 años con cuadro clí-nico de 6 meses de evolución de astenia, adinamia, dolor generalizado, pérdida progresiva de peso, síntomas depresivos y disfunción sexual con antecedente de vasectomía, quien fue valorado por múltiples especialidades, recibió tratamiento con antidepresivos y benzodiacepinas que produjeron mejoría parcial de la sintomatología. Posteriormente, de acuerdo con recientes criterios del Colegio Americano de Reumatología (ACR) se diagnosticó fibromialgia asociada a bajos niveles séricos de testosterona libre y globulina transportadora de hormonas sexuales.En la literatura no hay evidencia rotunda que correlacione traumas testiculares o posteriores a vasectomía con aparición de disfunción sexual, disminución de la relación testosterona libre -globulina transportadora de hormona sexual masculina y posterior aparición de fibromialgia en hombres. De igual forma, dada la progresiva aparición de este síndrome en hombres, se hace necesario aumentar su sospecha diagnóstica, descartando siempre otras patologías mediante exámenes clínicos y paraclínicos, haciéndose imprescindible que estos pacientes tengan un abordaje multidisciplinario por reumatólogos, fisiatras, urólogos y psiquiatras. rePorte de caso

All patients were evaluated for premenstrual syndrome (PMS) and primary dysmenorrhea (PD). Premenstrual syndrome was assessed among the patients for the presence of one or more affective or somatic symptoms within the five days preceding menses. The diagnosis of primary dysmenorrhea was defined as having abdominal pain or lower back pain lasting at least two days during a menstrual period.Dysmenorrhea was assessed via visual analog scale. Dysmenorrhea was rated via Multidimensional Scoring System. The Hamilton depression scale was applied to all patients.Results: Primary dysmenorrhea was established in 41% of FM patients and 28% of the control group. A statistically significant difference was found in PD between the two groups (p = 0.03).PMS was established in 42% of the FM patients and 25% of the control group. A statistically significant difference was found in PMS between the two groups (p = 0.03).Conclusion: There is an increased frequency of premenstrual syndrome and dysmenorrhea in FM patients. The patients with high symptom severity scores and high depression scores among the FM patients are at risk of PMS and PD.
