Objective: To explore sexual dysfunctions and expectations about sexuality in male patients with spinal cord injury (SCI) aged between 20-40. Material and Methods: A total of 75 traumatic SCI male patients were included in this study. The demographic and clinical characteristics were obtained by retrospective chart review. The differences between preinjury and post-injury sexuality status of the participants were assessed. The relationship between neurological level and erection/ejaculation was investigated. Results: The mean age of the participants was 26.16±4.35 years. The mean time since injury was 34.26±30.68 months. The number of the patients who had sexual interactions before injury was 63 (84%). Also, 34 patients (45%) had sex after injury, but 66 (88%) of the patients mentioned that their sexuality was affected after injury; 31 (41.3%) of the patients were not informed about sexuality in the post-injury period. Further, erection duration of 37 (60.6%) of the participants was shorter than 5 minutes, and 20 (32.8%) of the patients reported that they were satisfied with their erection. Moreover, 31 (41.3%) of the patients had reflex erection (RE), 14 (18.7%) of them had a psychogenic erection (PE), 16 (21.3%) of them got a mixed type of erection (RE+PE), and 14 (18.7%) of the patients did not have an erection. Ejaculation was present in 28 (37.3%) of the patients. The first three responses to questions about circumstances causing problems in post-injury sexual life and expectations for sexual life were erection, ejaculation, and orgasm, respectively. Conclusion: This study showed that sexual functions, like erection, ejaculation, and orgasm, in male patients were affected severely after SCI. The majority of the patients did not have enough sexual consulting, and the patients had great expectations for getting informed about sexuality in the post-injury period. Gereç ve Yöntemler: Çalışmaya 75 travmatik SKY'li erkek hasta dahil edildi. Hastaların tıbbi kayıtlarından seksüel rehabilitasyon takip dosyası elde edildi. Bu takip dosyası ile hastaların demografik ve klinik verileri ile yaralanma öncesi ve sonrası seksüel yaşamı tespit edildi. Nörolojik seviyelerin ereksiyon ve ejakülasyon ile olan ilişkileri araştırıldı. Bulgular: Hastalarımızın yaş ortalaması 26,16±4,35, yaralanma sonrası geçen süre 34,26±30,68 ay idi. Objective: To examine the association of neuropathic and nociceptive pain severity and interference with quality of life (QoL) in persons with spinal cord injury (SCI) who underwent a randomized controlled 12-week trial of an antidepressant to treat depression. A secondary objective was to assess the effect of changes in pain on mobility and physical independence. Design: Multivariable ANCOVA models controlling for relevant demographic covariates, treatment condition, and baseline pain and QoL were used. Setting: Six rehabilitation centers. Participants: Of the 133 persons who were randomized into the trial, 108 provided pain severity and interference ratings through follow-up. Interventions: Not applicable. Outcome Measures: The Satisfaction with Life Scale and the physical and mental component summary scores of the 12-Item Short-Form Health Survey (SF-12). Secondary outcome measures included the mobility and physical independence subscales of the Craig Handicap Assessment and Reporting Technique (CHART). Results: Broadly, few associations between pain and QoL were evident. Results revealed relationships between lower baseline nociceptive pain interference and higher satisfaction with life and mental health-related QoL at 12 weeks. Similarly, lower neuropathic pain interference was associated with change in physical independence, but unrelated to mobility. Conclusions: Pain interference over time may be differentially related to QoL outcomes based on the type of pain following SCI, but overall, there were no extensive relationships between pain and QoL in this sample of depressed persons with SCI.

Study design: Retrospective study. Objectives: Spinal cord injuries are usually associated with anejaculation, but few premature ejaculations (PE) are described in lumbosacral lesions. The objective of the study is to analyze the retrospective files of patients with lumbosacral lesions to explore whether PE is systematic, and which type of lesions (conus teminalis, epiconal, cauda equina) is associated with these PE. Methods: Standard sexological consultations from 34 men with L5-S4 lesions were analyzed including questions on psychogenic and reflexogenic erection, and ejaculation. Medical assessment included bulbocavernosus, anal, and cremasteric reflexes and urodynamics. Results: Thirty one (31/33) patients maintained natural ejaculations, but 18 complained of PE and five of spontaneous ejaculations. Fourteen patients complained of dribbling ejaculation, and 27 of non-climactic ejaculation (13 no sensation, 10 some sensation, 4 painful sensation). Medical assessments showed absent or diminished anal sensation in 28 patients, absent or diminished anal reflexes in 21, absent or diminished bulbocavernosus reflexes in 20, but 12/13 positive cremasteric reflex. Urodynamics showed 12/20 areflex and 2/20 hyperactive bladders. Conclusions: Most men with lower lumbosacral lesions maintain natural ejaculation, but often complain of PE following injury. The PE is such that a mere sexual thought can trigger the response, or appear spontaneously as daytime ('sticky paints') or nighttime ('sticky sheets') emissions. PE appears to be a direct consequence of the lesion as it is reported from the very first ejaculation post injury, in men who otherwise controlled their ejaculation prior to injury. Underlying mechanisms may involve sacral inhibition on ejaculation, or simultaneous activation of psychogenic erection and emission. Clinical implications are discussed.
