Serum methylprednisolone levels following intra-articular injection of methylprednisolone acetate.

Abstract: SUMMARY Twenty-one patients with rheumatoid arthritis received injections of either 40 mg or 80 mg of methylprednisolone acetate into one or both knee joints. Serum methylprednisolone and cortisol levels were measured at intervals up to 1 week following injection. Peak serum levels of methylprednisolone were reached at between 2 and 12 hours following injection, and increasing the injected dose resulted in correspondingly higher serum levels. Injection of 80 mg of methylprednisolone as 40 mg into each knee pro… Show more

“…The rate of absorption and duration of action are related to solubility of the compound, and triamcinolone hexacetonide is the most insoluble preparation currently available (16). Depending on the formulation, a steroid can be detected in synovial fluid up to 14 days after injection (29).…”

“…Enfin, il existe un effet anti-inflammatoire systémique des infiltrations secondaire au passage systémique du produit corticoïde [30], expliquant que l'effet thérapeutique ne se limite pas au site d'injection, l'état des articulations inflammées s'améliorant parallèlement à celle infiltrée.…”

“…However, plasma cortisol concentrations remain suppressed for up to a week following intra-articular steroid (Armstrong et al, 1981), and if the mechanism involved enzyme induction, for instance, duration will be dependent upon the half-life of the protein or proteins involved.…”
