SEOM clinical guidelines for endometrial cancer (2017)

Abstract: Endometrial cancer (EC) is the most common gynecological cancer in developed countries. Most patients are diagnosed at an early stage with a low risk of relapse. However, there is a group of patients with a high risk of relapse and poor prognosis. Despite the recent publication of randomized trials, the adjuvant treatment of high-risk EC is still to be defined and there are many open questions about the best approach and the right timing. Unfortunately, the survival of metastatic or recurrent EC is short, due … Show more

“…Endometrial cancer treatment often involves surgical removal of the uterus, ovaries, uterine tubes, and pelvic and paraaortic lymph nodes [40]. In the case of reproductive-aged women, fertility-sparing treatment is possible, however, it requires further standardization and development in order to provide the patient with access to various solutions [41,42]. It is important to look for complementary molecular markers that allow for an earlier and more precise diagnosis, as it can provide more treatment options and allow for the development of strategies that will be as non-invasive as possible for the patient.…”

“…Therapy selection and management protocol is closely related to the type, stage and grade of endometrial cancer, however, it is still not fully accurate. The Cancer Genome Atlas Research Network (TCGA) proposed an additional division of endometrial cancer into four molecular subtypes: POLE ultramutated, microsatellite instability hypermutated, copy number low, and copy number high, which emphasizes the high heterogeneity of this cancer [42]. Therefore, the classification of endometrial cancer taking into account changes at the molecular level is very important.…”

“…Nonetheless, this may reinforce the therapeutic benefit of LND specifically in advanced bulky nodal disease. For stage III EC, the ESMO-ESGO-ESTRO 2016 (5) supported by the newer SEOM guidelines (40) recommend radical cytoreductive surgery (debulking), which is defined as reducing the volume of a tumour to the greatest extent possible (41) . The Gynaecologic Oncology Group (GOG) recommends reducing gross residual disease to [?…”

“…Surgical Method: Minimally invasive surgery (MIS) is preferred over laparotomy based on ESMO-ESGO-ESTRO 2016 (5,(36)(37)(38) while laparotomy is preferred for complex cases (39) . Within MIS, robotic and laparoscopic LND shows similar adequacy (38)(39)(40) while robotic has certain advantages over laparoscopic surgery (38,41,42) . Role of sentinel lymph node (SLN) dissection: ESMO-ESGO-ESTRO recognises SLN algorithms as a potential alternative to systematic LND in early stage EC (5,43) supported by recent studies (23,36,44) .…”
