Sarcomatoid carcinoma and carcinosarcoma of the urinary bladder

Abstract: Two cases, sarcomatoid carcinoma and carcinosarcoma, of the urinary bladder are reported. A 68-year-old man with sarcomatoid carcinoma underwent total cystectomy and was alive and had had no recurrence after 21 months. A 78-year-old woman with carcinosarcoma underwent total cystectomy, but she died from increasing multiple lung metastases 4 months after surgery. The histopathological characteristics of both neoplasms are reported and discussed.

“…Polipoid, ülsere ve ilk tanıda muskuler tabakayıda invaze edecek şekilde ileri evrede olurlar (8), olgumuz soliter, 5 cm çapında, solid ve arka duvar kubbe bileşiminde yerleşimliydi. Tümörün agresif natürü, lokal rekürrense eğilimi nedeniyle sıklıkla cerrahi sonrası lokal rekürrens görülmesine rağmen radikal sistektomi ile adjuvan radyoterapi ve/veya kemoterapi kombinasyonu rehberlere net olarak gimemekle beraber önerilmektedir (9). Mesane sarkomatoid tümörü ortalama 17,2 ay gibi düşük sürviye sahiptir.…”

“…Diagnosis of carcinosarcoma of the bladder is usually made using conventional hematoxylin and eosin stained sections, especially when an easily identifiable carcinoma component is present in addition to the sarcomatous component. Although it is occasionally difficult to make a diagnosis, immunohistochemical analysis is of particular value in these tumors [6][7][8][9] . The carcinomatous component is epithelial marker-positive, while the sarcomatous component is negative, and it shows sarcomatous differentiation such as osteosarcoma, chondrosarcoma and rhabdomyosarcoma.…”

“…Hensley et al 15) reported that a 53% response rate was obtained with a combination therapy of gemcitabine, docetaxel and irradiation in unresectable leiomyosarcoma patients. Neoadjuvant chemotherapy was also a possible therapy, but we considered the possibility of tumor rapid growth and that the prognosis has been reported to be better in some reports [5][6][7][8]10) . Therefore, we performed radical cystectomy in a localized phase and achieved complete resection.…”

“…This type of tumor can occur in patients with a history of radiotherapy or chemotherapy. [7,8] Meyer (1920) classified carcinosarcomas into three histogenetic groups: 1) collision tumors in which carcinoma and sarcoma arise simultaneously in proximity and subsequently invade each other, 2) combination tumors in which both tumors derive from pluripotent cells, and 3) composition tumors in which the components arise from the same tissue. [9] Willis restricted the term carcinosarcoma by excluding collision tumors.…”
